URETHRAL DUPLICATION CASE PRESENTED WITH VOIDING DYSFUNCTION IN A GIRL

Introduction: Urethral duplication is a rare anomaly predominantly in males. It is very rare in girls but it should be considered. A case of complete ureteral duplication in a girl is presented because it is a rare anomaly which is published in only 26 cases in English literature.

Case report: 6-year-old girl who had tethered cord operation a year before has admitted to our department with the complaints of hesitancy and intermittent voiding. At physical examination, a pin-size hole was observed 2 cm above the clitoris that produce a small amount of fluid and the external genitalia was normal.  Contrast media was injected through this hole under scopy control and contrast media was observed to fill the bladder through a narrow and long canal. Cystoscopy was performed through the orthotopic urethral orifice, and  both ureters were observed in normal location and formation. The patient was diagnosed as a variant of complete Type 2 dorsal urethral duplication according to Stephens classification. The accessory urethra was in an epispadiac location relative to the ortotopic urethra. An incision was made around the hole and dissected down toward the bladder neck to the inferior aspect of the pubic symphysis and the tract which had narrowed markedly was amputated at this level. Dissection was continued through Phannenstiel incision, accessory urethra was found at the12 o’clock position, anterior aspect of the bladder wall, sharply dissected and transected below the symphysis. Histologic examination of the tract revealed squamous and transitional epithelium with smooth muscle. In early postoperative period, hesitancy and interrupted voiding pattern on uroflowmeter was disappeared.  The patient was discharged with uneventful recovery on postoperative 4th day. At 4 months her only complains were overactive bladder symptoms which are managed with medication.

Discussion: In girls with urethral duplication, treatment is indicated if the lesion is symptomatic and excision is usually curative. 

İŞEME BOZUKLUĞU İLE BAŞVURAN BİR KIZ OLGUDA ÜRETRA DUPLİKASYONU

Giriş: Urethral duplikasyon genellikle erkeklerde gözlenen nadir bir anomalidir. Kızlarda çok nadir görülmesine rağmen ayırıcı tanıda düşünülmelidir. Kızlarda tam uretral duplikasyon çok ender görülmektedir ve İngilizce literatürde 26 olgu bildirilmiştir.

Olgu Sunumu: Bir yıl önce gergin omurilik tanısıyla operasyon geçirme öyküsü olan 6 yaşındaki kız olgu, idrar yapmaya başlamada zorluk ve kesintili işeme şikayeti ile kliniğimize başvurmuştur. Fizik muayenesinde klitorisin 2 cm üzerinde damla tarzında ıslaklığa neden olan toplu iğne ucu büyüklüğünde bir orifis saptanmış ve diğer eksternal genital organlar normal bulunmuştur. Bu orifisten kontrast madde verilerek elde edilen skopik görüntülerde kontrast maddenin ince ve uzun bir kanalla mesaneye dolduğu izlenmiştir. Hastanın native üretra orifisinden girilerek yapılan sistoskopide her iki üreter orifisinin normal şekil ve lokalizasyonda olduğu izlenmiştir. Hastaya Stephens sınıflamasına göre Tip 2 tam dorsal üretra duplikasyonu varyantı tanısı konulmuştur. Aksesuar üretra ortotopik üretraya göre epispadic pozisyondadır. Orifisin etrafından yapılan insizyonla aksesuar uretra simfizis pubisin alt ucundan mesane boynuna kadar disseke edilmiş ve daraldığı görülerek bu seviyede bırakılmıştır. Operasyona Phannenstiel insizyonla devam edilip mesane önünde saat 12 hizasında aksesuar üretra bulunmuştur. Mesaneye girdiği bölgeden keskin diseksiyonla ayrılarak simfizin altına kadar takip edilip aksesuar üretranın  mesane ve deriye açılan uçları çıkarılmıştır. Histolojik incelemede skuamoz ve transizyonel epitel ve düz kas hücreleri gözlenmiştir. Operasyon sonrası erken dönemde idrar yapmaya başlamada zorluk ve üroflowmetrede kesintili işeme paterninin kaybolduğu görülmüştür. Post operatif 4.gün komplikasyonsuz olarak taburcu edilmiştir. Operasyonunun 4.ayında olan hastanın tek şikayeti aşırıaktif mesane semptomlarıdır ve  ilaç tedavisi ile kontrol altında tutulmaktadır.

Tartışma: Üretral duplikasyon kızlarda nadir görülen bir anomali olup semptomatik ise cerrahi eksizyon ile başarıyla tedavi edilebilir.
