Aim

Spina bifida is a congenital anomaly which necessitates life long treatment . Long term treatment can be achieved with strong parent doctor relationship in this patient group. The aim of this study was to open ways to create proper relation methods with the parents by determining the depression and anxiety level of them.

 

Patients and Methods

The parents of patients who admitted to our center with the diagnosis of spina bifida were included in the study. Beck Anxiety Inventory (BAI) and Beck Depression Inventory (BDI) were enrolled to the parents. They were asked to fill these forms together with sociodemographic data form.

 

Results

A total of 52 pair of parents were included in the study. Majority of the parents were observed to be primary school graduates. 54% of the families had a monthly income below 1000 TL. The most difficult  point of baring such a child was found to be emotional depression of the condition for the mothers and this point was economical difficulties for the fathers. Mother BAI was 17.2 +12.02 and this score was 10.2+5.65 for the fathers. Mother BDI was 15.4+2.82 and this score was 11.1+2.12 for the fathers.

 

Conclusions

Spina bifida is a disease of low income families in most of the cases. As mothers are more sensitive emotionally, they are more vulnerable for depression and anxiety. Psychological support programs are necessary to support families in chronic diseases to obtain treatment durability and consistency.

Amaç

Spina bifida yaşam boyu devam eden ve uzun süreli tedavi süreçlerine ihtiyaç duyan doğumsal bir hastalıktır. Çocuk ürolojisi pratiğinde karşılaştığımız bu hasta grubunda bu uzun süreli tedavi hekim ile ebeveynler arasındaki oluşmuş sağlam ilişki ile gerçekleştirilebilir. Bu çalışmanın amacı ebeveynlerin depresyon ve anksiyete düzeylerini tespit ederek uygun iletişim metodlarını geliştirmeye imkan sağlamaktır.

 

Hastalar ve Metod

Merkezimize spina bifida tanısı ile başvurmuş hastaların anne ve babaları çalışmaya alınmıştır. Ebeveynlere yönelik Beck Anksiyete Envanteri (BAE) ve Beck Depresyon Envanteri (BDE) uygulanmıştır.  Kendilerine verilen bu formları sosyodemografik veri formu ile beraber doldurmaları istenmiştir.

 

Sonuçlar

Çalışmaya 52 anne ve baba katılmıştır. Çalışmaya katılan anne ve babaların çoğunluğu ilkokul mezunu olduğu gözlenmiştir. Ailelerin %54’ünde gelir düzeyi aylık 1000 TL nin altındadır. Anne açısından çocuğun en çok yük getiren yönü çoğunlukla duygusal çöküntü olarak tanımlanmışken, babalar da ekonomik zorluklar olarak ifade edilmiştir. Anne BAE skoru 17.2 +12.02, babalarda 10.2+5.65 saptanmış, anne  BDE skoru 15.4+2.82, babalarda bu değer 11.1+2.12 olarak gözlenmiştir.

 

Tartışma

Spina bifida çoğunlukla düşük gelir düzeyindeki ailelerin çocuklarında görülen bir hastalıktır. Aile içinde annelerin duygusal açıdan daha hassas olmaları onları hem anksiyete hem de depresyon açısından daha kırılgan hale getirmektedir. Yaşam boyu süren bu kronik hastalıkta tedavi sürekliliği ve uyumu devamlı kılabilmek için başta anneler olmak üzere ebeveynlere yönelik psikolojik destek programlarına ihtiyaç duyulmaktadır.
