A SEVERE COMPLICATION OF CIRCUMCISION: URETHRAL COMPLETE OBSTRUCTION

~~Circumcision is excision of the preputium to reveal the glans penis.  Although, beyond the doubt, it is the most common procedure in our country, severe complications appear since this procedure was excluded from the scope by social security organizations for years and is performed by unauthorized people in inappropriate conditions. In the present study, we aimed to present a complication which is rare but also may cause serious outcomes.
Case
A 13 year old male patient was referred to our clinic due to the complaint of inability to urinate and prediagnosis of urethral stenosis by placing a cystofix catheter since no catheter could be placed into the urinary bladder.   It was learned that urination problem of the patient developed after circumcision at 6 years of age and current complaints increase during winter season. In the physical examination, urethral mea location and diameter were normal. In the urinary ultrasound, hydronephrosis and parenchyma thinning (tapering?) (2 mm) and thickening on urinary bladder wall (12 mm) were reported. Multiple parenchyma damage was detected on both kidneys in the scintigraphy. In the patient examination under anesthesia, a complete obstruction at approximately 1 to 2 cm was detected in the urethra. Urethra continuity did not exist in the cystopical exploration. No other orifice was observed in the gland. Current cystofix catheter was removed and anterograd cystoscopy was performed. Urinary bladder was severely dilated and trabeculated. When the guide was forwarded along the bladder neck to the distal side, easy forwarding and exiting from a small orifice at frenulum location were observed. The fistula was dilated over the guide and cystoscope was inserted; proximal side of the urethra was observed dilated. Meatotomy was performed and cystofix was removed after placing a 12 fr siliconized foley catheter into the urinary bladder. Foley catheter was removed at day 10. 1. In uroflowmeter taken in the postoperative 1 st month, Qmax was normal, no residue was detected; however, urinayr bladder capacity was calculated higher. The patient follow-up continues by providing a behavioral treatment.

Conclusion
Possible complications after circumcision do not cause cosmetic problems only but also severe problems which may progress to kidney failure if ignored or missed. Therefore, circumcision should be performed by competent surgeons under general anesthesia.

CİDDİ BİR SÜNNET KOMPLİKASYONU; ÜRETRAL TAM OBSTRÜKSİYON

Sünnet; prepisyumun, glans penisi ortaya çıkaracak şekilde eksizyonudur. Ülkemizde  hiç şüphesiz en çok yapılan ameliyat olmasına rağmen yıllarca sosyal güvenlik kurumlarınca kapsam dışında tutulması, yetkisiz kişilerce ve uygunsuz koşullarda yapılması bir takım ciddi komplikasyonlara yol açmaktadır. Bu çalışmada sünnete bağlı nadir ancak  ciddi sonuçlar doğurabilecek bir komplikasyonu sunmayı amaçladık.
Olgu
13 yaşında erkek hasta dış merkez idrar yapama şikayetiyle ve mesaneye sonda yerleştiremediği için sistofiks katater takılarak üretral darlık öntanısıyla kliniğimize sevk edildi. Hastanın işeme proplemlerinin 6 yaşında sünnet olduktan sonra geliştiği ve özellikle kış aylarında  mevcut şikayetlerinin arttığı öğrenildi. Fizik muayesinde  üretral mea normal lokalizasyonda ve çapta idi. Üriner USG’de özellikle sağ böbrekte hidronefroz ve parankimde incelme (2 mm) ve mesane duvar kalınlığında artış (12 mm)  raporlandı. Sintigrafisinde her iki böbrekte multipl  prankim hasarı tespit edildi. Hastanın anestezi altında muayesinde üretrada yaklaşık 1-2 cm mesafeden tam obstrüksiyon tespit edildi. Sistoskop ile bakıda üretranın devamlılığı yok idi. Glansta başka bir orifis  görülemedi. Mevcut sistofiks katateri çekilerek anterograd sistoskopi yapıldı. Mesane ileri derecede dilate ve trabeküle  idi. Mesane boynununa ilerlenip distale guide göderildiğinde kataterin rahat ilerlediği ve frenilum lokalizasyonunda küçük bir orifisten dışarı çıktığı görüldü. Fistül guide üzerinden dilate edilerek sistoskopla girildiğinde üretra proksimalinin dilate olduğu görüldü. Meatotomi yapılarak,  mesaneye 12 fr silikonize foley sonda konulup sistosiks çekildi. 10. gün  foley çıkartıldı. 1. ayında  üroflowmetride Qmax normal rezüdüsü yok ancak mesane kapasitesi yüksek olarak hesaplandı. Hastaya davranış tedavisi verilerek takiplerine devam edilmektedir.

Sonuç
Sünnet sonrası ulaşabilecek komplikasyonlar sadece kozmetik değil, eğer farkedilmez veya ihmal edilir ise böbrek yetmezliğine götürecek kadar ciddi sorunlar doğurabilmektedir. Bu nedenle sünnetin mutlaka genel anestezi ve işin ehli uzmanlarca yapılması önerilmektedir.
