Laparoscopic Treatment of Renal Hydatid Cyst Mimicking Renal Tumour with Diode LASER Technique: A Case Report

Introduction:

Cystic echinococcosis being one of the most important helmintic infections, is a serious life-treathening health problem in developing countries. Kidney is third most commonly effected organ after the liver and the lungs. The incidence of hydatid cyst was reported  as 3.4 /100.000 with only  % 2 renal involvement in our country. The diagnosis of hydatid cyst of kidney is generally put incidentally and can be misdiagnosed as primary kidney tumours. In addition, hydatid cyst of kidney is a more rare condition in children and can be treated with medical therapy. For resistant cases surgical treatment may be required. Here we present a case of renal hydatid cyst resistant to medical therapy who was treated with laparoscopic excison with diode laser.

Case Presentation:

A 15 year old female patient was admitted with abdominal pain. Abdominal ultrasonogpaphy revealed a 32*23*19 mm solid mass with cystic component at lower pole of right kidney. An indirect hemagglutination (IHA) test for echinococcosis granulosus was performed which was positive at a 1:320 titre. Other laboratory tests were within normal limits. The patient recieved albendazole therapy  for 3 months. Control magnetic resonance imaging showed a solitary leison with exophtic  extensions which was containing large sepatations. No  contrast enhancement  could be detected after gadalonnium injection.  As no regression could be detected radiologically surgical treatment was planned. Laparoscopic renal lower pole mass cyst excision with diode laser was performed and the patient was hospitalized for  one day without  any  blood transfusion need. Histopathological examination which was consistent with hydatid cyst of kidney confirmed the diagnosis. At  the postoperatvie second month the ultrasonographic imaging of kidneys were normal. 

Tümörü Taklit Eden Böbrek Kist Hidatiğinin Diode LASER yardımı ile Laparoskopik Tedavisi

Kistik ekinokokkozis en önemli helminto zoonoz enfeksiyonlardan birisidir.  Gelişmekte olan ülkelerde insan sağlığını tehdit eden önemli bir halk sağlığı sorunu olarak bilinmektedir. Üriner sistem tutulumu karaciğer ve akciğerlerden sonra en sık tutulan bölgedir. Ülkemizdeki insidansı 3.4/100000 olarak bildirilmiştir.  Böbrek kist hidatiği tanısı çoğunlukla rastlantısal olarak konulmakta ve ayırıcı tanıda primer böbrek tümörleri ile karıştırılabilmektedir. Böbrek kist hidatiği çocuklarda nadir olarak görülmektedir ve çoğunlukla medikal tedavi ile tedavi edilebilmektedir. Medikal tedaviye yanıt alınamayan olgularda cerrahi tedavi gerektirmektedir. 

Olgu Sunumu

15 yaşında bilinen herhangi bir hastalığı olmayan kız çocukta karın ağrısı nedeniyle yapılan USG’da sağ böbrek alt polde kist görülmesi üzerine  değerlendirildi.  Ultrasonografi’de sağ böbrek alt polde 32*23*19 mm boyutlarında kistik komponenti bulunan solid kitle olarak raporlandı. Ekinokokkus granulosis indirekt hemaglunitasyon serolojisi (İHA) 1/320 + olarak bulundu.  Diğer biyokimyasal parametreleri normal olarak saptandı.  Kist hidatik öntanısı ile aldendazol tedavisi başlandı. 3 kür aldendazol tedavisi sonrası yapılan MR görüntülemede; sağ böbrek alt polde ekzofitik uzanımlı, içerisinde kalın septalar bulunan, kontrast madde ile boyanmayan lezyon saptandı. Kistik kitlede herhangi bir radyolojik gerileme olmaması üzerine hastanın sağ böbrek alt pol kitle eksizyonu ile tedavi edilmesine karar verildi.

Videoda sunulduğu gibi laparoskopik sağ böbrek alt pol diode lazer yardımlı kitle eksizyonu yapıldı. Hasta 1 gün hospitalize edildi. Kan transfüzyonu yapılmadı.  Şifa ile taburcu edildi. Kitlenin histopatolojik inceleme sonucu kist hidatik ile uyumlu olarak raporlandı. Hastanın postoperatif 2. ayda yapılan USG’da sağ böbrek normal olarak raporlandı. Hasta albendazol tedavisi almaya devam ediyor.
