Hypospadiasis repair and liposuction for the obese patient with severe hypospadiais

AİM:

We aimed to represent the obese patient with penoscrotal hypospadiasis and excessive pubic fat who underwent pubic liposuction and had the penile shaft evident.

 

CASE REPORT:

Obese patient with penoscrotal hypospadiasis had hypospadiasis repair gradually. Besides the methods of degloving, plication and penopubic sling pubic liposuction were performed to be able to make the penile shaft evident.

SURGİCAL TECHNIQUE:

Pubic lipectomy and liposuction are seldomly preferred methods for buried penis. Degloving, plication, chordee release, penopubic sling and z plasty were performed. The length of the penile shaft were detected and the inguinal skin greft was sutured on to the penil shaft. Finally liposuction were performed from the inguinal area with a 3mm canula.

 

RESULT:

For the patients with severe hypospadiasis when the cosmetic result is not satisfactory after the urethral meatus is carried to the anatomical place; pubic liposuction can be added to the classical methods.

DİSCUSSİON.

 Liposuction can be a choice for patients with severe hypospadiasis and excessive pubic fat for satisfactory results of cosmesis.

AMAÇ:

Penoskrotal hipospadıaslı pubik yağlanması olan obez hastada pubik liposuction yardımı ile penil şaftı ortaya cıkardıgımız vakayı sunmayı amacladık.

 

OLGU SUNUMU:

Opere penoskrotal hipospadıaslı obez olgunun hıpospadiasının kademeli olarak düzeltilmesini takinem penil şaftı  daha görünür kılmak icin degloving, plikasyon, penopubik askı yöntemlerine ek olarak  pubik liposuction yapıldı .

 

AMELİYAT TEKNİĞİ:

Pubik lipektomi ve liposuction gömülü penis tedavisinde nadiren tercih edilen bir tekniktir. Hastaya degloving, penopubik askı, plikasyon, chordee release ve z plasti işlemleri yapıldı.  Penil şaft uzunluğu ortaya çıkarıldıktan sonra inguinal bölgeden alınan cilt grefti penil şafta sütüre edildi. Ardından 3 mm lik kanül ile inguinal bölgeden girilerek liposuction yapıldı.

SONUÇ:

Ağır hipospadiaslı vakalarda üretral mea anatomik lokalizasyona getirilmesine ragmen tatmin edici görüntü sağlanamadıysa pubik liposuction klasik yöntemlere  eklenebilir

TARTIŞMA

 Ağır hipospadiaslı hastalarda pubik yağlanma da mevcutsa kozmetik olarak daha tatminkar sonuçlar sağlamak için liposuction seçenek olabilir.
