MİTCHELL'S TECHNİQUE FOR EPİSPADİAS REPAİR

Epispadias
associated bladder extrophy or isolated is a congenital malformation with a spesific
surgical correction. In 1996, Mitchell published his technique known by his
name and the epispadiac phallus is completely disassembled into three parts in
this technique.It’s based in the
complete dissection of the uretheral band, ventral peneal skin and both
corporas cavernosa. It permits to: 1)Correct the dorsal incurvation and chordee; 2)Locate the
urethra in a ventral position; 3)Locate the meatus at the tip of the penis with
a vertical orientation. We present a video of one-year-old case who was
performed Mitchell’s technique for penopubic epispadias and we performed to
correct the bladder extrophy when he was 2-day-old. In conclusion; Mitchell’s
technique allows an anatomic reconstruction of the penis with good cosmetic
appearance and functional results, although it is an agresive technique than
other techniques.
EPİSPADİAS ONARIMINDA MİTCHELL TEKNİĞİ

Epispadias, izole yada mesane ekstrofisi ile birlikte özellikli cerrahisi olan konjenital malformasyondur. Mitchell 1996’da kendi adıyla anılan, epispadiak fallusun 3 parçaya ayrıldığı tam bir penil ayrışma tekniğini sunmuştur. Üretral plağın , ventral penil cilt ve iki kavernöz cismin birbirinden tamamen serbestleştirilmesi temeline dayanan tekniğin bize sağladığı avantajlar: 1)Düzgün bir dorsal oyukluk sağlanması ile dorsal kordinin düzeltilmesi 2)Üretranın ventral pozisyonda olmasının sağlanması 3) Üretral meanın penis ucunda ve vertikal planda oluşturulmasıdır. Biz 2 günlükken ekstrofi vezika nedeniyle mesane onarımı yaptığımız penopubik tip epispadiaslı 1 yaşındaki olgunun Mitchell tekniğini kullanarak epispadias onarımı sırasındaki operasyon videosunu sunuyoruz. Sonuçta diğer tekniklere göre daha agresif bir teknik olmasına rağmen epispadias onarımında Mitchell tekniği iyi kozmetik ve fonksiyonel sonuçlar sağlar.
