The Clinical Significance of Hydronephrosis With or Without Vesicoureteric Reflux in Children with Spina Bifida

IntroductionThe primary goal of treatment for neurogenic bladder is to protect renal function. The aim of this study was to identify the clinical significance of hydronephrosis w/o vesicoureteric reflux (VUR) in children with spina bifida.Patients and MethodsMedical records of patients with spina bifida were evaluated. Those patients who had hydronephrosis were identified and divided into two groups; Group 1, patients with VUR and Group 2, without VUR. Age, gender, urodynamic parameters, urinary tract infection history, renal scarring, side of lesion, and presence of associating genitourinary anomaly were noted. Parameters were evaluated with chi-square and Student’s t test.ResultsOut of 1200 patients who were on follow up for spina bifida, 41 patients were identified with hydronephrosis. Only patients who had a complete diagnostic work up were included. There were 17 patients in Group 1 and 17, in Group 2. The mean age was 5,23,5 years in Group 1 and 6,95,6 years in Group2. The comparison of gender distribution as well as urodynamic parameters, infection history, side of lesion and associating anomaly were insignificant. Only patients who had hydronephrosis without VUR had significant renal scarring compared to those with VUR Conclusions Predicting the risk factors for renal scarring in neurogenic bladder dysfuntion and its prevention is an important goal in spina bifida. Hydronephrosis without VUR seems to be an important risk factor for renal scarring in these patients. The cause of scarring should be delineated well for the prevention of this unwanted side effect of neuropathic bladder in children with spina bifida.
