Clinical results in nonpalpable testes

Aim: To compare results of operation according to open or laparoscopic dissection of funiculus spermaticus for nonpalpable testes.

Patients and methods: Patients referred or consulted to our clinic for nonpalpable testes between January 2010-December 2015. 59 patients examined due to diagnosis of nonpalpable testes Eight patients had bilateral nonpalpable testes, 67 testicular unit took place in this study. Eight testes were atrofic and testicular unit was ended blind, then excluded. So, 59 testicular unit were operated. Mean age of patients was 44.7 months. Patients were grouped ans group 1, operated with open surgery and group 2, operated laparoscopically. There were 18 (39.5%) and 41 (60,5%) testicular units in group 1 and group 2, respectively.

Results: In group 1, orchiopexies were done successfully without any tension in nine testicular units (50%). Two (11.1%) of them were positioned at midlevel of canal and seven (38.9%) an high scrotal position. In group 2, 25 testes (61%) were placed into scrotum without any tension and the remained testes (39%) were at the high scrotal position at the end of the operation.

Conclusion: Laparoscopy provides some advantages for diagnosis end management for many diseases but for nonpalpable testes, there is no evident advantage.

Palpe edilemeyen testislerde ameliyat sonuçlarımız

Amaç: Palpe edilemeyen testislerde funikulus spermatikus diseksiyonunun açık ya da laparoskopik yapılmasına göre ameliyat sonuçlarının karşılaştırılması

Hastalar ve yöntem: Ocak 2010-Aralık 2015 tarihleri arasında polikliniğimize başvurmuş ya da yönlendirilmiş hastalar çalışmaya dahil edildi. 59 hasta palpe edilemeyen testis tanısı ile değerlendirildi. 8 hasta iki taraflı palpe edilemeyen testis mevcuttu. Toplamda 67 testiküler ünite muayene sonuçlarına göre çalışmada yer aldı. 8 testiküler ünitenin atrofik olduğunun ve funikulus spermatikus yapılarının kör sonlandığının görülmesi üzerine çalışmadan çıkarılarak kalan 59 ünite değerlendirildi. Hastaların yaş ortalaması 44,7 ay idi. Açık yöntemle ameliyat edilenler grup 1, laparoskopik olarak ameliyat edilenler grup 2 olarak sınıflandırıldı. Grup 1' de 18 (%39,5), grup 2' de 41 (%60,5) testiküler ünite yer aldı.

Bulgular: Açık olarak ameliyat edilenlerin 9 tanesi (%50), funikulus spermatikus gergin olmayacak şekilde skrotuma yerleştirilebildi. Geri kalanların funikulus spermatikus yapılarının gerginliğinden dolayı 2 tanesi (%11,1) inguinal kanal ortasına, 7 tanesi (%38,9) ise yüksek skrotal pozisyona indirilebildi. Grup 2' dekilerin 25' si (%61) yine funikulus spermatikus yapılarında herhangi bir gerginlik olmayacak şekilde skrotuma indirilebilirken, 16 (%39) tanesi yüksek skrotal pozisyona yerleştirildi.

Sonuç: Laparoskopi kullanımı birçok ameliyatta tanı ve tedavi açısından açısından avantajlı olmasına rağmen palpe edilemeyen testislerde, testisin bulunması ve skrotuma indirilmesi açısından belirgin bir üstünlük tespit edilememiştir.
