Comparison of outcomes of laparoscopic varicocele ligation and microscopic varicocelectomy in preadolescent and adolescent boys

Introduction: Surgical treatment of varicocele is controversial in prepubertal boys: Open approach (microscopy asisted or not), laparoscopic. 

 

Methods: We compared two groups of prepubertal and pubertal patients who underwent surgical treatment for varicocele in our clinic between 2003 and 2015, retrospectively. Group 1 included boys who underwent laparoscopic unilateral varicocele ligation (LVL)  and group 2 included patients who underwent unilateral microscopic varicocelectomy (MV). We compared the operative time, outcomes and complications between groups. Fisher’s exact test,  Mann-Whitney U test and independent samples t test were used for statistical analysis.

 

Results: We identified a total of 43 patient (mean age was 13,2 years), 30 in group 1 and 13 patients in group 2. Varicocele were detected on the left side in 40 patients and right in 3 cases. There was no difference between groups in terms of age, side and grade of varicocele at presentation. Median operative time in group 1 and 2 were 55 min (35-70 min) and 50 min (40-80 min), respectively. Scrotal hematoma was seen in one cases in MV group, and hydrocele were detected in 3 cases in LVL group. Postoperative impairment in testiculer volume were detected in four cases, 3 in group 1 and 1 in group 2. There was no postoperative wound infection or reccurence in both groups. There was no statistically significant difference between groups in terms of outcomes and complications.

 

Conclusion: Although there were longer operative time, higher hydrocele ratio and more cases with postoperatively decreased testiculer volume in LVL group, there was no statistically significant difference between groups. The findigs of our study demostrated that both procedures were effective options in correction of preadolescent and adolescent varicocele.

 

Preadölesan ve adölesan hastalarda laparoskopik varikosel ligasyonu ile mikroskobik varikoselektomi sonuçlarının karşılaştırılması

Giriş

Özellikle prepubertel ve pubertal erkeklerde varikoselin cerrahi tedavisi halen tartışmalıdır: Açık cerrahi ( mikroskobik veya değil) veya laparoskopik yaklaşım.

Metod

Kliniğimizde varikosel nedeniyle 2003 ile 2015 yılları arasında tedavi edilmiş prepubertal ve pubertal hastalar geriye dönük olarak değerlendirildi. Grup 1; laparoskopik tek taraflı ligasyon (LVL) uygulanan çocuklar, grup 2; tek taraflı mikroskopik varikoselektomi (MV) uygulanan hastalar olarak belirlendi. Operasyon süresi, sonuçları ve komplikasyonları karşılaştırıldı. Fisher’s exact test, Mann- Whitney U test ve bağımsız örneklem T testi istatistik analizi için kullanıldı.

Bulgular

Yaş ortalaması 13,2 yıl olan 43 hastanın verileri değerlendirildi.  Varikosel 40 hastada solda, 3 hastada sağ taraftaydı. Grup 1’de 30, grup 2’de 13 hasta vardı. Gruplar arasında yaş, taraf, varikosel derecesi açısından fark yoktu. Grup 1’de ortalama operasyon süresi 55 (35-70) dakika iken, grup 2’de 50 (40-80) dakika saptandı. Skrotal hematom sadece MV grubunda bir hastada görülürken, postoperatif hidrosel LVL grubunda 3 hastada görüldü. Postoperatif testis volümü azalması grup 1’de 3 hastada, grup 2’de 1 hastada olmak üzere toplam 4 hastada gözlendi. Yara yeri enfeksiyonu veya rekürrens her iki grupta da görülmedi. Gruplar arasında sonuçlar ve komplikasyonlar açısından istatistiksel olarak anlamlı fark yoktu.

Sonuç

LVL grubunda, daha uzun operasyon süresi, daha yüksek postoperatif hidrosel oranı ve daha fazla sayıda hastada testis volüm kaybı görülmüş olmasına rağmen istatistiksel olarak gruplar arasında fark bulunmamıştır. Her iki yöntemin de preadölesan ve adölesan hastalarda varikosel tedavisinde effektif tedavi seçenekleri olduğunu düşünmekteyiz.
