Ureteroneocystoctomy in the management of ureterocele

Aim: Upper pole heminephrectomy became the most preferred approach for ureterocele, as upper pole constitutes a small percentage of total renal function and to avoid bladder surgery. We prefer ureterocele excision and ureteroneocystoctomy even for non-functioning moieties to avoid potential complications of retained ureterocele and need for secondary intervention. The aim of this study was to evaluate outcomes of patients operated for ureterocele.

Patients-Methods: Hospital records of patients who had undergone surgery for ureterocele and vesicoureteric reflux – as a control group - between years 1994-2015 were reviewed retrospectively.  Data including demographics of patients, radiological findings, procedures performed, complications, postoperative urinary tract infection (UTI) and bladder function were evaluated.

Results: Fourty two patients were operated in our clinic because of ureterocele. 34 of the ureteroceles were unilateral (R/L: 22/12) and 8 bilateral.  Ureterocele incision was performed in all patients. Subureteric injection was indicated for the other pole ureter  in 4 and for post incision vesicoureteric reflux during followup in 1 patient. Eighteen patients underwent ureteroneocystostomy (including 4 patients with nonfunctional upper poles) and 3 heminephrectomy. One of the patients in heminephrectomy group required secondary ureteroneocystostomy for UTI.

Conclusion: Urinary tract infections and bladder dysfunction was observed in the minority of our patients following ureteroneocystostomy for ureterocele. Ureterocele excision and ureteroneocystostomy is a safe and effective procedure in the management of ureterocele with no requirement of secondary interventions.

 

Üreterosel tedavisinde üreteroneosistostomi

Amaç: Çift toplayıcı sistemde üst kutbun toplam fonksiyona katkısının az olması ve mesane cerrahisinin komplikasyona daha açık olması öngörüsüyle tüm dünyada üreterosel tedavisinde üst kutup heminefrektomisi en çok tercih edilen yöntem haline gelmiştir. Kliniğimizde ise kalan üreterosele bağlı gelişebilecek komplikasyonlardan kaçınılması ve ikincil cerrahi gereksiminin azaltılması için işlevsiz üst kutba sahip olgulara dahi ilk tercih olarak üreterosel eksizonu ve üreteroneosistostomi uygulanmaktadır. Bu çalışmada üreterosel nedeni ile üreteroneosistostomi uygulanmış olguların postoperatif komplikasyonlar açısından değerlendirilmesi amaçlanmıştır.

Olgu - Yöntem: 1994-2015 yılları arasında üreterosel nedeni ile opere edilen olgular (grup 1) ve kontrol grubu (grup 2) olarak vezikoüreteral reflü nedeni ile opere edilmiş olguların dosyaları geriye dönük taranmıştır. İkincil nedenlere bağlı gelişen vezikoüreteral reflüye sahip olgular çalışma dışı bırakılmıştır. Veriler demografik özellikler, radyolojik bulgular, operasyon şekli, komplikasyonlar, postoperatif idrar yolu enfeksiyonu (İYE) ve mesane fonksiyonları açısından incelenmiştir.

Sonuçlar: Kliniğimizde 42 olgu (K/E: 26/16) üreterosel nedeni ile opere edilmiştir. 34 olguda unilateral (sağ/sol: 22/12), 8 olguda bilateral üreterosel mevcuttur. Tüm olgulara ilk girişim olarak üreterosel insizyonu uygulanmıştır. Subüreterik enjeksiyon, 1 olguya üreterosel insizyonu sonrası VUR gelişmesi üzerine ve 4 olguya insizyon ile aynı seansta alt kutup üreterine uygulanmıştır. Onsekiz olguya üreteroneosistostomi (4 işlevsiz kutup üreterine dâhil), 3 olguya heminefrektomi uygulanmıştır. Bunun yanısıra heminefrektomi uygulanan olgulardan birine izlem sırasında VUR sebebiyle İYE görülmesi üzerine üreteroneosistostomi uygulanması gerekmiştir

Yorum: Öngörülenin aksine olgularımızda üreteroneosistostomi sonrası idrar yolu enfeksiyonu ve mesane disfonksiyonu az sayıda olguda gözlenmiş, hiçbir olguya ikincil müdahale gerekmemiştir. Üreterosel tedavisinde üreterosel eksizyonu ve üreteroneosistostomi güvenilir ve etkin bir yöntemdir.
