Experiences with unilateral Lich Gregoire operation in children

Aim: To evaluate outcome after unilateral Lich Gregoire operation (LG) by open approach in children.Patients and method: The medical records of 68 patients undergoing unilateral LG by open approach between 2000-2015 were analyzed retrospectively. The indications, complications and outcome of urinary tract infections (UTI) were abstracted and 49 patients (Male: 31, female: 18) met these criteras. Results: The associated anomalies were contralateral agenezi/dysplazia (n:7), neurogenic bladder (n:5), ipsilateral ureteropelvic junction obstruction (n:4), contralateral low grade vesicoureteral reflux (n:5), posterior urethral valv (n:2), urolithiasis. Fourteen patients were on anticholinergics and, 6 on CIC preoperatively. Operation was indicated for ureterovesicale junction obstruction (UVJ) in 28 patients (after subureteric injection: 2, reimplantation: 1), vesicoureteral reflux (VUR) in 18 (after failed subureteric injection: 4) and UVJ obstruction plus VUR in 3. Unilateral LG ( left: 30, right: 19) was performed at a mean age 4,5 years (r: 6 months- 17 years). Forty-one patients underwent resection of distal ureter and reanastomosis and 3 ureteral infolding (JJ: 38, external catheters: 3). The ureter was single in 40 patients, two in 6 and ectopic in 3. Mean follow-up was 3,8 years ( r: 1-10). Neither had increased postoperative hydronephrosis nor secondary interventions. Of 31 patients with UVJ obstruction, hydronephrosis was decreased in 28. The complications (n:5, 10%) were low grade VUR (n:3), diverticula and decreased renal function after ectopic ureteral reimplantation. Contralateral low grade VUR resolved in 5 of 3 (60%) patients. The incidence of preoperative and postoperative UTI was 69% (34/49) and 34% (17/49). Conclusion: Unilateral Lich Gregoire operation by open approach is an effective technique for the treatment of ureterovesicale junction obstruction and vesicoureteral reflux in children, has the advantages of establisment of adequate submucosal tunnel, ureteral reimplantation without angulation, can also be performed in duplicated collecting systems/neurogenic/valve bladder and as a redo uretereoneocystostomy.
