Labial fusion: a new classification for the management

Objective: to determine the subtypes of the labial fusion (LF) and to arrange treatment options according to the types.

Material and Methods: patients who were admitted to Baku Oksigen Hospital and Safa Hospital due to LF between July 2016 and February 2018 were divided into 4 groups. The location of the fusion area, the thickness of the fusion tissue, and the response to topical steroid therapy were identified as common features.

Findings: A total of 50 patients were included. The mean reference age was 16 (3-84) months. The most frequent cause of the admittion was the fusion detected in the routine examination, while 20% of the patients complained of disturbance while urinating. The disease divided into 4 types according to their common characteristics;

Type I (48%): fusion tissue is thin, the fine feeding probe (FFP) can be inserted between the fusion tissue and the hymen. Reflection of the hymen or feeding probe can easily be visualised. Fusion heals with topical steroid therapy within two weeks(100%).

Type II (20%): fusion tissue is thick, the FFP can be inserted between the fusion tissue and the hymen. Reflection of the hymen or feeding probe can not be visualisedThe response to topical steroid treatment is taken approximately 3 weeks(80%).

Type III (24%): fusion tissue is thick, the FFP can not be inserted between the fusion tissue and the hymen. The fusion tissue continues to the front of hymen. Unresponsive to topical steroid treatment (100%).

Type IV or Interlabial Type Fusion(8%): fusion between labia minor and major. Responsive to topical steroid treatment(100%).

Conclusion: Classifying LF patients in this way provides a major contribution to the treatment of patients by a pre-determined strategy. We have not seen superiority of topical estrogen therapy to corticosteroid treatment in our patients.

Amaç: Labial füzyon'un alt tiplerinin belirlenmesi ve tedavi seçeneğinin tiplere göre düzenlenmesidir.

Materyel ve Metod: temmuz 2016-şubat 2018 arasında labial füzyon nedeni ile Bakü Oksigen Hastanesi ve Özel Safa Hastanesine başvuran hastalar ortak özelliklerine göre gruplara ayrıldı. Ortak özellik olarak; füzyon alanının yeri, füzyon dokusunun kalınlığı ve füzyonun topikal steroid tedavisine cevabı belirlendi.

Bulgular: Toplam 50 hasta çalışmaya dahil edildi. Ortalama başvuru yaşı 16 ( 3-84) ay olarak belirlendi. En sık başvuru sebebi rutin muayenede saptanan füzyon iken, hastaların %20'si idrar yaparken huzursuzluk şikayeti ile başvurmuştu. Hastalık ortak özelliklerine göre 4 tipe ayrıldı;

Labial Füzyon Tip I (%48): füzyon dokusu ince, ince beslenme sondası füzyon dokusu ile himen arasına ilerletilebiliyor. Himen'in veya beslenme sondasının reflesi kolaylıkla görülebilmektedir. İki haftalık topikal steroid tedavisi ile iyileşme sağlanmaktadır (%100).

Labial Füzyon Tip II (%20): füzyon dokusu kalın, ince beslenme sondası füzyon dokusu ile himen arasına ilerletilebiliyor. Himen'in veya beslenme sondasının reflesi görülememektedir. Topikal steroid tedavisine yanıt ortalama 3 haftada alınmaktadır (%80). Eğer 3. haftasını doldurmuş topikal steroid tedavisine yanıt yoksa cerrahi separasyon tercih edilmelidir. (%20).

Labial Füzyon Tip III (%24): füzyon dokusu kalın, ince beslenme sondası füzyon dokusu ile himen arasına ilerletilememektedir. Füzyon dokusu himen'in önüne kadar devam etmekte, bazen himen'in kenarlarını da tutmaktadır. Topikal steroid tedavisine yanıtsız (%100).

Labial Füzyon Tip IV ve ya İnterlabial Tip Füzyon (%8): Labia Minor'lar ile labia major'lar arası füzyon. Hastalar vajinal akıntı veya kötü koku nedeni ile başvurmaktadır. Topikal steroid tedavisine yanıtlı (%100).

Sonuç: Labial füzyon hastalarını bu şekilde sınıflandırmak hastaların tedavisinin deneme-yanılma ile değil, baştan belirlenmiş strateji ile düzenlenmesinde büyük katkı sağlamaktadır. Hastalarımızda topikal östrojen tedavisinin kortikosteroid tedavisine üstünlüğü görülememiştir ve yan etkiler nedeni ile tercih edilmemesi gerektiğini düşünmekteyiz.
