Giant Cervical Polyp Rarely Seen In Children:Vaginoscopic Excision

Aim: Giant cervical polyps are rare entities, with only 9 reported cases in the international literature. Of these cases, only two have been described in the pediatric population. This video was intended to present vaginoscopic excision and technical details in a child with a giant cervical polyp. 

Case: A 5-year-old girl was admitted to our clinic with vaginal bleeding for 2 months and a prolapsed mass from the last 2 days. The laboratory results of the patient had no features other than mild leukocytosis (15,990 uL). The pelvic USG as '' A 40x20 mm cystic mass in the posterolateral adjasent of the bladder, urethral prolapse ?'', was reported . Under anesthesia, a mass prolapsed from vagina  was detected. The urethra and bladder were normal in the cystoscope. Hysterosalpingography showed normal uterus and tubes. On the vaginoscope, a mass originating from the right lateral wall of the external os of the cervix and which  filling the vagina was detected. The mass was ligated by turning the pedicle two turns with 3/0 suture held in forceps in the vaginoscope. The remaining part was held with forceps and cut out with cautery. The patient who had no problems during and after the surgery, was discharged the 2nd day. Peroperative and postoperative course was uneventful and she was discharged on postoperative 2 th day. The pathology was reported as benign polyopoid lesion without atypia and mitotic activity.

 Conclusion: A giant cervical polyp that is rarely seen in children can be vaginoscopically removed with a minimally invasive procedure without deteriorating hymen integrity. However, it should be kept in mind that this age group may be cervical-derived rhabdomyosarcoma.

Çocuklarda Nadir Görülen Dev Servikal Polip: Vajinoskopik Eksizyon

Amaç: Dev servikal polipler nadir olup, uluslararası literatürde sadece 9 vaka bildirilmiştir. Bu vakalardan sadece ikisi pediatrik yaş grubunda tariflenmiştir.  Bu videoda dev servikal polibi olan bir çocuk hastada vaginoskopik eksizyon ve teknik detayların sunulması amaçlandı.

Olgu: 5 yaşında kız hasta, 2 aydır süre gelen vaginal kanama ve son 2 gündür vestibulumda prolabe kitle nedeniyle kliniğimize başvurdu. Hastanın laboratuvar sonuçlarında hafif lökosiztoz (15.990 uL) dışında özellik yoktu. Pelvik USG’si ⹂“mesane posterolateral komşuluğunda 40x20 mm kistik kitle, üretral prolapsus ?” olarak raporlandı. Anestezi altında muayenede vajenden prolabe olan kitle tespit edildi. Sistoskopide üretra ve mesane , histerosalpingografide uterus ve tubalar normal idi. Vajinoskopide; vajeni dolduran, serviks ağzı sağ lateral  duvardan kaynaklanan kitle tespit edildi. Kitle, vaginoskop içerisinde forsepsle tutulan 3/0 sütür ile  pedikülü iki tur dönülerek bağlanıp çıkartıldı. Kalan kısım forseps ile tutulup koterle kesilip çıkartıldı. Ameliyat sırasında ve sonrasında problemi olmayan hasta 2.gün taburcu edildi. Patolojisi atipi ve mitotik aktivite göstermeyen benign poliopid lezyon olarak raporlandı.

Sonuç: Çocuklarda nadir gözlenen dev servikal polip, himen bütünlüğü bozulmadan minimal invaziv bir yöntemle vajinoskopik olarak çıkartılabilir. Ancak bu yaş grubunda serviks kaynaklı rabdomyosarkom olabileceği akılda tutulmalıdır.
