A CASE OF RETROPERITONEAL IMMATURE TERATOMA TOTALLY EXCISED WITH SEGMENTAL VENA CAVA RESECTION AND KIDNEY SPARING APPROACH

Background/Aim: Immature teratomas are rarely seen childhood germ cell tumors that rarely invade to surrounding tissues. Here, we aimed to discuss the surgical approach for a giant sized retroperitoneal immature teratoma with vena cava invasion.

Case: A 76-day-old female patient was applied to hospital for abdominal swelling after 2 weeks from birth, have a computer tomography scan showed a mass with a solid and cystic component containing widespread calcifications and fat densities, extending to the left side of the abdomen, passing through the midline and completely filling the right lumbar area. The patient was operated with preliminary diagnosis of retroperitoneal tumor. During exploration, a giant mass originated from the right retroperitoneal area and compressing kidney was found. Vena cava was displaced, markedly compressed and partly invaded by the mass till the subhepatic area. The mass was totally excised with suprarenal segmental excision of vena cava, while preserving the right kidney and renal vein. Then vena cava was anastomosed end-to-end. In the postoperative period, low molecular weight heparin (LMWH) treatment was started. Serial Doppler ultrasound examinations revealed that vena cava flow was normal and the thrombus was not observed. The patient was discharged on the 11th postoperative day. Histopathologic examination was reported as immature teratoma. The patient was consulted by pediatric oncology and chemotherapy was not administered her. LMWH treatment was terminated 1.5 month after surgical operation. There was no recurrence at postoperative 7th month and vena cava integrity was found as normal.

Conclusion: Retroperitoneal immature teratomas invading vena cava can be treated with a surgical approach that maintains vascular integrity and spares kidney. When appropriate anticoagulant strategy is applied after the surgery, morbidity associated with vena cava thrombosis may be prevented postoperatively.

SEGMENTAL VENA KAVA REZEKSİYONU VE BÖBREK KORUYUCU YAKLAŞIMLA TOTAL OLARAK EKSİZE EDİLEN BİR RETROPERİTONEAL İMMATÜR TERATOM OLGUSU

Ön bilgi/Amaç: İmmatür teratomlar çocuklarda nadir görülen ve çevre dokulara ender olarak invazyon yapan germ hücreli tümörlerdir. Burada vena cava invazyonu yapan ve dev boyutlu olan bir retroperitoneal immatur teratom olgusuna uygulanan cerrahi yaklaşımın tartışılması amaçlanmıştır.

Olgu: 76 Günlük kız hastanın, doğumdan 2 hafta sonra gelişen karın şişliği yakınmasıyla başvurduğu dış merkezde çekilen bilgisayarlı tomografisinde sağ lomber alanı tümüyle dolduran ve orta hattı geçerek karın sol tarafına da uzanım gösteren, içerisinde yaygın kalsifikasyonlar ve yağ dansiteleri içeren solid ve kistik komponentleri olan kitle saptandı. Merkezimize gönderilen hasta retroperitoneal tümör ön tanısı ile ameliyat edildi. Ameliyatta sağ retroperitoneal alandan köken alan ve böbreğe belirgin bası yapan dev boyutlu bir kitle görüldü. Sağ renal veni de içerecek şekilde kitle tarafından itilmiş ve sarılmış olan vena kava inferior, subhepatik alana kadar kitle içinde invaze olarak seyrediyordu. Sağ böbrek, renal ven ve infrarenal vena kava diseke edilerek korundu. Kitle içinde seyreden suprarenal vena kava bölümü ile beraber total olarak eksize edildi. Ardından vena kava uç uca anostomoz edildi. Postoperatif dönemde hastaya düşük molekül ağırlıklı heparin (DMAH) tedavisi başlandı. Doppler ultrasonografi incelemelerinde vena kava akımı normal bulundu ve trombüs izlenmedi. Hasta postoperatif 11. günde taburcu edildi. Histopatolojik inceleme sonucu immatür teratom olarak rapor edildi. Çocuk onkolojisi tarafından değerlendirilen hastaya kemoterapi uygulanmadı. DMAH tedavisi 1.5 ay devam edildikten sonra sonlandırıldı. Postoperatif 7. ayda yapılan kontrolünde rekürrens saptanmamış olup vena kava devamlılığı normal bulunmuştur.

Sonuç: Vena kava invazyonu yapan retroperitoneal yerleşimli immatür teratom olguları vasküler bütünlüğü devam ettirici ve böbreği koruyucu bir cerrahi yaklaşımla tedavi edilebilir. Cerrahi girişim sonrasında uygun antikoagulan stratejisi uygulandığında, postoperatif dönemde vena kava trombüsü ile ilgili morbidite oluşumu engellenebilir.
