A Serious Complication of the Circumcision: Urethral Fistula

Introduction: The aim of this study is to present the clinical features and treatment outcomes of the patients who developed urethral fistula after circumcision.

Patients and Methods: Files of cases operated for urethral fistula developed after circumcision between 2015-2019, age, the person who performed circumcision, anesthesia, circumcision technique, the place where circumcision was performed, the fistula localization, number of fistulas, age of the fistula repair, the fistula repair technic, stenting, duration of hospitalization and complications were evaluated retrospectively.

Results: Six patients had fistula. The average age of the patients was 4.3 years (ranged between 3-6 years). Three of the cases were circumcised by local circumciser, 2 by medical officer and 1 by pediatric surgeon. 5 of the circumcision was performed in the home environment and 1 in the hospital. All circumcisions were performed under local anesthesia. Five of the cases were guillotine and one of was performed by dorsalslit technique. Only one case had cautery. The average period between the dates of referral to our clinic and circumcision was 4 months. Coronary and glanular fistulas was developed in one patient and coronal was developed in five cases. Fistula treatment was performed 6-9 months after the circumcision. Stent was implanted in all cases. The mean duration of hospitalization of the cases was 7 days. Recurrence was developed none of the patients.

Conclusion: The urethral fistula was developed frequently after the circumcision performed by local circumciser or medical officer with guillotine technique under local anesthesia.

Sünnetin Ciddi Bir Komplikasyonu: Üretral Fistül

Giriş: Sünnet sonrası üretral fistül gelişen olguların klinik özellikleri ve tedavi sonuçlarının sunulması amaçlandı.

Hastalar ve Yöntem: 2015-2019 yılları arasında sünnet sonrası gelişen üretral fistül nedeniyle opere edilen olguların dosyaları yaş, sünneti yapan kişi, anestezi, sünnet tekniği, sünnetin yapıldığı yer, fistül lokalizasyonu, fistül sayısı, onarım yaşı, onarım tekniği, stent, hastanede yatış süresi ve komplikasyon açısından geriye dönük değerlendirildi.

Bulgular: Toplam altı olguda fistül gelişmişti. Olguların ortalama yaşı 4.3 (3-6 arasında değişiyordu) yıldı. Olguların 3’ü mahalli sünnetçi, 2’si sağlık memuru, 1’i çocuk cerrahisi uzmanı tarafından sünnet edilmişti. Sünnetlerin 5’i ev ortamında, 1’i hastanede yapılmıştı. Tüm sünnetler lokal anestezi altında yapılmıştı. Olguların 5’i giyotin, bir tanesi dorsalslit tekniğiyle yapılmıştı. Sadece bir olguda koter kullanılmıştı. Kliniğimize müracaat tarihleri ile sünnet olma tarihleri arasında geçen ortalama süre 4 aydı. Beş olguda koronal düzeyde bir olguda hem koronal hem glanüler düzeyde fistül gelişmişti. Olgulara sünnetten 6-9 ay sonra fistül onarımı yapılmıştı. Tüm olgulara stent yerleştirilmişti. Olguların ortalama hastanede kalma süresi 7 gündü. Olguların hiç birinde nüks gelişmedi.

Sonuç: Üretral fistül sıklıkla ev ortamında mahalli sünnetçi veya sağlık memuru tarafından lokal anestezi altında, giyotin tekniğiyle yapılan sünnetlerden sonra gelişmişti.
