Vertical plication: a new penil curvature correction technique that avoids penile shortening and reduces the need for urethral plate transection in penoscrotal hypospadias

Aim:Whilst the low grade penil curvatures(PCs) can be corrected by penile degloving, high grade PCs are usually demand more complex surgi­cal techniques. The Nesbit and tunica albuginea plication techniques have served satisfactory results at the cost of penile length. Also, elongating the penis by grafting the concave side has the long-term complications of graft contraction and erectile dysfunction. We here describe an incisionless and safe “vertical plication” technique.

Methods:Buck’s fascia at the maximum point of curvature was incised longitudinally at 12-o’clock position and then dissected from tunica albuginea medial to lateral in each side. No incisions made through the tunica albuginea. Corporal rotation was performed by approximating the 3 to 5 polyester sutures which were placed 5 mm apart at and around the point of maximum curvature. We detected narrower appearance at the side of vertical plication, which was resolved by de-epithelialized vascular flap coverage harvested from foreskin.

Results:The median age of the patients was 2 years(range:1-4 years). The location of hypospadias was penoscrotal in 16 patients and only one patient had congenital PC without hypospadias. Full correction was achieved in 15 patients, and residual curvature below 10 degree was seen in two patients. The median pre-plication and post-plication artificial erection angle was 750(range:50–1300) and 00(range:0–80), respectively (p < 0.001). Following rotation, penile length was measured 0.29±0.59 mm (median:0 mm, range:0–2 mm) longer (p = 0.059). We were able to perform single-stage repair in 13 (81,25%) of them with the aid of “vertical plication” technique.

Conclusion:“Vertical plication” technique makes the correction of PC easier, effective and less invasive. It avoids the penile shortening that may be caused by incision of tunica albuginea and dorsal plication. It also allows the surgeon to proceed with single stage repair in patients with proximal hypospadias associated with high grade PC.

Vertikal plikasyon: penoskrotal hipospadiasta penisin kısalmasını önleyen ve üretral plate transeksiyon gereksinimini azaltan yeni bir penil eğrilik düzeltme tekniği

Amaç:Düşük dereceli penil eğrilikler penisin soyulması ile düzeltilebilirken, yüksek dereceli eğrilikler genelde daha kompleks cerrahi teknikler gerektirirler. Nesbit ve tunika albuginea plikasyon teknikleri tatmin edici sonuçlar sunmasına rağmen penisin kısalmasına neden olmaktadırlar. Bununla birlikte, penisin konkav tarafının greftlenerek uzatılmasının greft kontraksiyonu ve erektil disfonksiyon gibi uzun dönem komplikasyonları vardır. Biz bu yazıda insizyonsuz ve güvenli bir yöntem olan “vertikal plikasyon” tekniğini tariflemekteyiz.

Yöntem:Buck fasyası eğriliğin en fazla olduğu noktada longitidunal olarak (saat 12 hizasından) insize edildi. Her iki tarafta tunika albuginea medialden laterale doğru diseke edildi. Tunika albuginea insize edilmedi. Maximum kurvatürün olduğu alanda, 5 mm.lik aralıklarla 3-5 adet polyester sütür kullanılarak korporal rotasyon tamamlandı. Vertikal plikasyon bölgesinde oluşan dar görünüm, sünnet derisinden alınan deepitelize vasküler flep kullanılarak düzeltildi.

Bulgular:Hastaların ortanca yaşı 2 yıl(aralık 1-4 yıl) idi. On altı hatada penoskrotal hipospadias mevcuttu. Sadece 1 hastada hipospadias olmaksızın konjenital penil eğrilik vardı. On beş hastada tam düzeltme sağlanırken, sadece 2 hastada 100nin altında eğrilik kaldı. Ortanca pre-plikasyon ve post-plikasyon artifisyal ereksiyon açısı sırasıyla 750(aralık: 50–1300) ve 00(aralık: 0–80) idi (p< 0.001). Plikasyon sonrası, penis 0.29±0.59 mm(ortanca:0 mm, aralık:0–2 mm) daha uzun olarak ölçüldü (p = 0.059). Penoskrotal hipospadiası olan hastaların 13’ünde (81,25%) vertikal plikasyon tekniği sayesinde tek seans ile onarım yapılabildi.

Sonuç:“Vertikal plikasyon” tekniği, penil eğriliğin daha kolay, efektif ve daha az invaziv olarak düzeltilmesini sağlar. Tunika albuginea insizyonu ve dorsal plikasyonun neden olabildiği penis kısalmasını önler. Ayrıca yüksek dereceli penil eğriliği olan proksimal hipospadiaslı hastaların tek seansta onarılmasına olanak sağlar.
