46 XY GONODAL DYSGENESIS, CASE REPORT

INTRODUCTION

Primary amenorrhea occurs in about 2% of women of reproductive age. Although the most common cause is gonadal dysgenesis, it also occurs due to congenital absence of uterus and vagina and constitutional delay. It should not be forgotten that patients with gonadal dysgenesis have an increased risk of malignancy of approximately 30% with increasing age.

CASE

A 16-year-old female looking patient applied to the adolescent outpatient clinic due to primary amenorrhea. On physical examination, breast development was compatible with her age. XY gonadal dysgenesis was considered in the evaluation of the patient. Estradiol levels was found to be low in the examinations performed by the endocrinology. The patient was started on estradiol treatment. The patient started menarche. Pelvic USG reported as “Both ovaries were observed as small for the age of the patient and band shaped. Chromosome analysis was performed, and the result was 46 XY, the patient was diagnosed with 46 XY gonadal dysgenesis (Swyer syndrome). Pelvic MRI reported as “There is an antherverted uterus in the midline. Myometrium is homogeneous. Corpus-cervix separation is formed. Formed ovarian tissue with follicles was not observed in both adnexal regions. No distinctive male or female gonad structures were identified in either inguinal region or canal.”. Afterwards, laparoscopic exploration was performed, and in the perioperative evaluation of the patient, it was observed that the gonads were band-shaped and bilateral gonadectomy was performed. The patient's pathology result was gonadoblastoma for both gonads.

CONCLUSION

Bilateral gonadectomy is recommended as soon as the diagnosis is made due to the risk of malignancy in patients with Swyer syndrome who present with the complaint of primary amenorrhea. Gonadoblastoma and dysgerminoma are the most common. It is recommended to start cyclic estrogen and progesterone therapy in order to prevent the effects of estrogen deficiency in postoperative follow-ups.

GİRİŞ

Primer amenore, üreme çağındaki kadınların yaklaşık %2’sinde görülür. En sık neden gonadal disgenezi olmakla birlikte uterus ve vajenin konjenital yokluğu ve konstitüsyonel gecikme nedeniyle de karşımıza çıkar. Gonadal disgenezi saptanan hastalarda yaş ilerledikçe artan yaklaşık %30 malignensi riski olduğu unutulmamalıdır. Bu olgumuzda, primer amenore etyolojisi araştırılırken 46 XY gonodal disgenezi (Swyer sendromu) olduğu anlaşılan, bilateral gonadektomi yapılan ve patoloji sonucu gonadoblastom olarak raporlanan hastamızdan bahsedeceğiz.

OLGU

16 yaş female görünümde hasta, primer amenore nedeniyle adölesan polikliniğine başvurmuş. Hastanın öyküsünde ek hastalığı, aile öyküsü ve bilinen başka major anomalisi olmadığı öğrenildi. Fizik muayenesinde meme gelişimi yaşıyla uyumluydu. Hastanın yapılan değerledirilmesinde XY gonadal disgenezi düşünülmüş. Çocuk endokrinoloji tarafından yapılan tetkiklerinde östradiol düşük saptanmış. Hastaya östradiol başlanmış. Hastada menarş başlamış. Pelvik usg: “Her iki over hastanıın yaşına göre küçük boyutlarda bant tarzında izlendi.” şeklinde raporlanmış. Hasta genetiğe konsülte edilerek kromozom analizi yapılmış, 46 XY olarak sonuçlanması sonrası 46 XY gonadal disgenezi (Swyer sendromu) tanısı almış. Çekilen pelvik mr: “Orta hatta antervert yerleşimli uterus bulunmaktadır. Myometrium homojendir. Korpus-serviks ayrımı oluşmuştur. Her iki adneksiyal bölgede üzerinde follikül bulunduran forme over dokusu izlenmedi. Her iki inguinal bölgede ve kanalda belirgin dişi ya da erkek gonad yapısı seçilmedi.” şeklinde raporlanmış. Sonrasında tarafımızca laparoskopik eksplorasyon yapılan hastanın perioperatif değerlendirilmesinde gonadların bant şeklinde olduğu görülerek bilateral gonadektomi yapıldı. Hastanın patoloji sonucu her iki gonad için de gonadoblastom şeklinde sonuçlandı.

SONUÇ

Primer amenore şikayeti ile başvuran, Swyer sendromu saptanan hastalarda, malignite riski nedeniyle tanı konulur konulmaz bilateral gonadektomi önerilmektedir. En sık gonadoblastom ve disgerminom görülmektedir. Postoperatif takiplerinde östrojen eksikliğinin etkilerinden korunmak amacıyla hastalara siklik östrojen ve progesteron tedavisi başlanması önerilmektedir.
