INCOMPLETE URETHRAL DUPLICATION IN THREE MALE CHİLDREN

İntroduction: Urethral duplication (UD) is a rare congenital anomaly with numerous anatomical variants and various presentations.In this study, we present three cases operated for urethral duplication.

Case 1: A 10-year-old male patient presented with the complaint of a second urethral opening approximately 2mm above the normal urethral meatus on the dorsal penis.Urinary ultrasonography and VCUG were normal.In the operation, it was observed that it was not related to the normal urethra, and ended blindly after approximately 1-1.5cm, and it was totally excised.

Case 2: A 4-year-old male patient presented with the complaint of double urethral opening and difficulty in urination. He had glandular hypospadia.There was also a normal urethral opening.Urinary ultrasonography was normal. Before the operation, cystourethroscopy was performed.It was observed that the normal urethral meatus had a blunt end at approximately 2cm.The meatus, which was in the form of a small fistula at the glandular level, was enlarged and it was seen that it reached the bladder by cystoscopy.While the hypospdias was repaired, both urethra were joined to ensure the integrity of the urethra.

Case 3: A 8-year-old male patient presented with a second urethral opening at the dorsum of the penis, a dorsal cordee and penile left rotation. In the cystourethroscopy performed during the surgery, it was observed that the second urethra, located on the dorsum of the penis, ended blindly at the pubic bone alignment. Blind-ending urethra was totally excised, dorsal cordee and penile rotation were corrected. The patients had no postoperative complaints. According to the Efmann classification, 3 patients had Type 1 A:incomplete urethral duplication anomaly.

Conclusions: Treatment of urethral duplication depends on the anatomy and clinical manifestations of the duplication. Since the prognosis is variable depending on the type and accompanying anomalies, these should be considered when planning surgical treatment.

ÜÇ ERKEK ÇOCUKTA İNKOMPLET ÜRETRAL DUPLİKASYON

Giriş: Üretral duplikasyon (UD), çok sayıda anatomik varyantı ve çeşitli sunumları olan nadir bir konjenital anomalidir. Bu çalışma ile , üretral duplikasyon nedeniyle ameliyat edilen üç vakayı sunuyoruz.

Olgu1: 10 yaşında erkek hasta ;penis dorsalinde nomal üretral meanın yaklaşık 2 mm üzerinde ikinci bir üretral açıklık şikayeti ile başvurdu. Üriner ultrasonografisi ve VCUG normaldi. Ameliyat sırasında normal olan üretra ile ilintili olmadığı yaklaşık 1-1,5 cm sonrasında kör sonlandığı görüldü ve total eksize edildi.

Olgu 2: 4 yaşında erkek hasta ,çift üretral açıklık ve idrar yaparken zorlanmak şikayeti ile başvurdu. glandüler hipospadias ve normal üretral çaıklık şeklinde çift mea görüldü. Üriner ultrasonografisi normaldi . ameliyat öncesinde sistoüretroskopi yapıldı normal olan üretral meanın yaklaık 2.cm de kör sonlandığı görüldü. Glandüler seviyefe küçük bir fistül şeklinde olan mea genişletildi ve sistoskopi ile mesaneye ulaştığı görüldü. Hipospdias onarımı yapılırken her iki üretra birleştirilerek üretra bütünlüğü sağlandı.

Olgu 3: 8 yaşında erkek hasta , penis dorsumunda ikinci bir üretral açıklık; dorsale doğru cordee ve penil sol rotasyon şikayeti ile başvurdu. ameliyat sırasında yapılan sistoüretroskopide penis dorsumunda bulunan ikinci üretranın pubik kemik hızasında kör sonlandığı görüldü. Kör sonlanan üretra total eksize edildi dorsal cordee ve penil rotasyon düzeltildi. Hastaların ameliyat sonrası şikayetleri olmadı . efmann sınıflamasına göre 3 hastada tip 1 a üretra duplikasyonu anomalisi mevcuttu.

Conclusions: Üretral duplikasyonun tedavisi, duplikasyonun anatomisine ve klinik belirtilerine bağlıdır. Prognoz, tipe ve eşlik eden anomalilere bağlı olarak değişken olduğundan cerrahi tedavi planlanırken bunlar dikkate alınmalıdır.
