VESICOURETERAL REFLUX DUE TO THE MUCOSAL BRIDGE OCCURING BETWEEN BOTH URETERIC ORIFICES AFTER OPEN ANTEREFLUX SURGERY

Introduction:

Vesicoureteral reflux (VUR) is one of the most common pathologies that cause urinary tract infections in childhood. The location and the angle of the ureteral orifices play an important role in the pathophysiology of VUR. It aims to present a rare acquired pathology, the mucosal bridge secondary to open antireflux surgery formed between both ureteric orifices caused by VUR.

Case: A nine-year-old male patient presented with abdominal and flank pain, dysuria, and hematuria. The patient had been treated for VUR at the age of one in Iraq by open surgery, but the type of the surgery is unknown and probably was LBP. On urinary USG, the right kidney was grade 2-3 and the left was grade 2 hydronephrotic. Left grade-4 VUR was detected in voiding cystography. In DMSA, the contribution of the left kidney was calculated as 80% and the contribution of the right kidney as 20%. The left kidney was in normal size and function; the right kidney had decreased cortical functions, due to cortical defect areas observed in the upper and lower pole. Cystoscopy revealed that the left ureteric orifice was slightly superolateral to the trigone. It was thought to be grade-4 VUR due to the insufficiency of the submucosal tunnel created in the previous operation. In addition, it was observed that there was a band-shaped mucosal adhesion starting from under the left ureteral orifice and extending towards the right ureteral orifice. After the band excision, it was observed that both ureteral orifices relocated to the normal anatomical position. Double HIT injection with two ccs Dexell was performed for treatment of the left grade 4 VUR.

Conclusion: It should be kept in mind that inter orificial bands may cause secondary VUR as a very rare condition after open antireflux surgery.

AÇIK ANTİREFLÜ CERRAHİSİ SONRASI GELİŞEN HER İKİ ÜRETERİK ORİFİS ARASINDA OLUŞAN MUKOZAL KÖPRÜYE BAĞLI GELİŞEN VEZİKOÜRETERAL REFLÜ

Giriş:

Vezikoüreteral reflü (VUR), çocukluk çağında en sık idrar yolu enfeksiyonuna sebep olan patolojilerden biri olup üreter orifislerinin mesaneye giriş lokalizasyonları ve açısı VUR fizyopatolojisinde önemli rol oynamaktadır. Nadir gözüken bir patoloji olan; VUR’a neden olan her iki üreterik orifisin arasında oluşan mukozal köprünün sunulması amaçlanmıştır.

Olgu:

Dokuz yaşında yabancı uyruklu erkek hasta karın ağrısı, yan ağrısı, idrarda yanma ve hematüri şikayeti ile başvurdu. Özgeçmişinde 1 yaşındayken Irak'ta VUR ve bilateral hidronefroz tanısı ile açık ameliyat yapıldığı sözel olarak öğrenildi. Üriner USG'de sağ böbrek grade 2-3, sol böbrek ise grade-2 ektazik görünümde idi. Voiding sistografide sol grade-4 VUR saptandı.DMSA'da sol böbreğin katkısı %80, sağ böbreğin katkısı ise %20 olarak hesaplanmış olup sol böbrek normal boyut ve fonksiyonda; sağ böbrek ise kortikal fonksiyonları azalmış, üst polde ve alt dış kesimde kortikal defekt alanları izlenmiş şekildeydi. Sistoskopide sol üretra orifisinin trigonun hafif superolateralinde olduğu izlendi. Daha önce geçirdiği operasyonda oluşturulan submukozal tünelin yetersizliğine bağlı grade-4 VUR olduğu düşünüldü. Ayrıca sol üreter orifisi altından başlayıp sağ üreter orifisine doğru uzanan belirgin bant şeklinde mukozal yapışıklık olduğu izlendi, bu mukozal çekintinin de reflü sebebi olabileceği düşünüldü ve bugbee koter yardımıyla bant eksize edildi. Bant kesildikten sonra her iki üreter ağzındaki çekintinin kaybolduğu ve rahatladığı görüldü. Ayrıca 2 cc double hit enjeksiyon yapıldı.

Sonuç:

Nadir bir durum olarak VUR ameliyatı sonrası her iki üreter meası arasında mukozal bant oluşması durumunda sekonder vezikoüreteral reflü gelişmesi akılda bulundurulmalıdır.
