RARE CASE OF CLOACAL MALFORMATION WITH COMPLEX UROGENITAL ANATOMY

AİM Cloacal malformation (CM) is a rare member of then rectal malformation group. Some of the cases of CM may present with complex anatomy and severe urogenital anomaly. Due to the different variations in urogenital anomalies, various challenging factors may be encountered in the management of these cases. This study aims to present a rare case of cloacal malformation with complex urogenital anatomy and to discuss it in current literature.

CASE: A female patient who was followed up due to the suspicion of bilateral hydronephrosis and anal atresia in the antenatal period and diagnosed as cloacal malformation upon the detection of a single urogenital opening in the postnatal evaluation was diagnosed with the diagnosis of rectal atresia and discrete colostomy procedure at the age of two days implemented. Evaluation of the genital system revealed bilateral hydroureteronephrosis, pelvic ectopic right kidney and bifid vagina. In the case where the common canal length was measured as approximately 1 cm in the cystoscopy, a separate bladder and vagina entrance could not be seen. Extra ectopic kidney, ureteric fibrotic band with ectopic location in the pelvis that cannot be clearly visualized in preoperative examinations in the definitive laparotomy performed on the patient who was followed up with colostomy+ left ureterocutanotomy + uterostomy. The patient underwent cystectomy, ileocystoplasty + urinary diversion (mitrafanoff) + colon pull through in the same session. The patient whose vesicostomy and colostomy status are still ongoing and follow-up with the plans to close the vesicostomy and colostomy.

DISCUSSION AND CONCLUSION Cloacal malformation is a group of congenital anomalies that affect the anorectal and urogenital system to varying degrees, which are difficult to manage clinically not only operatively but also in the long term. Long-term management and follow-up is recommended for the maintenance of both the intestine and the bladder.

KARMAŞIK ÜROGENİTAL ANATOMİSİ İLE NADİR GÖRÜLEN KLOAKAL MALFORMASYON OLGUSU

GİRİŞ:Kloakal malformasyon (KM), anorektal malformasyon grubunun nadir görülen üyesidir.KM olgularının bazıları karşımıza karmaşık anatomi ve ciddi ürogenital anomali birlikteliği ile çıkabilir. Ürogenital anomalilerdeki farklı varyasyonlar nedeniyle bu olguların yönetiminde çeşitli zorluklar ile karşılaşılabilir. Bu çalışma karmaşık ürogenital anatomisi ile nadir görülen kloakal malformasyon olgusunu sunmayı ve güncel literatür eşliğinde tartışılmasını amaçlamaktadır.

OLGU:Antenatal dönemde iki taraflı hidronefroz ve anal atrezi şüphesi nedeniyle takibi yapılan ve postnatal yapılan değerlendirmede tek ürogenital açıklık tespit edilmesi üzerine kloakal malformasyon tanısı alan kız hastaya rektal atrezi tanısı ile iki günlükken ayrık kolostomi işlemi uygulandı. Ürinogenital sisteme yönelik değerlendirmede iki taraflı hidroüreteronefroz, pelvik ektopik yerleşimli sağ böbrek ve bifid vagen tespit edildi. Sistoskopik değerlendirmede ortak kanal uzunluğu yaklaşık 1 cm olarak ölçülen olguda, ayrı bir mesane ve vagen girişi görülemedi. İki taraflı üreter orifisleri bulunamaması nedeniyle iki taraflı perkütan nefrostomi işlemi uygulandı.Kolostomi + sol üreterokutanostomi + uterostomi ile takip edilen hastaya yapılan definitif laparotomide ameliyat öncesi tetkiklerde net olarak görüntülenemeyen pelvis içinde ektopik yerleşimli pelvisi geniş, üreteri fibrotik bant şeklinde ekstra ektopik böbrek dokusu olduğu görüldü. Hastaya aynı seansda sistektomi, ileosistoplasti + üriner diversiyon (mitrafanoff) + kolon pull trough işlemi yapıldı. Vezikostomi ve kolostomi durumu devam eden hastanın vezikostomi ve kolostomi kapatılması planlanı ile takibi devam etmektedir.

TARTIŞMA VE SONUÇKloakal malformasyon, anorektal ve ürogenital sistemi farklı derecelerde etkileyen, sadece operatif olarak değil, aynı zamanda uzun vadede klinik açıdan da yönetilmesi zor konjenital anomali grubudur. Hem bağırsak hem de mesane bakımı için uzun süreli yönetim ve takip önerilmektedir.
