Parameatal urethral cyst in adolescence

Introduction: Parameatal urethral cyst is a rare clinical condition that may cause impaired urinary flow or be asymptomatic and cause cosmetic concerns. It may resolve spontaneously or require surgical excision.

Case: A 12-year-old patient presented with complaints of diffuse urination and a cyst on the urethral meatal margin for 3 years. On physical examination, a spherical cyst of 1 cm in diameter, narrowing the meatus, was detected in the external urethral meatus. It was learned from his history that there was no trauma and infection. Surgical excision was planned, since growth was observed in the size of the cyst in the follow-up. During the excision of the cyst, no connection was observed between the cyst and the urethra. Its pathology was reported as a benign cystic lesion. No recurrence was observed in the follow-up.

Conclusion: Parameatal cyst is formed due to the defective process of separation of the foreskin from the glans. These cysts usually appear at birth or early childhood and may occur at the ventral or lateral edge of the urethral meatus. The diagnosis is made according to the clinical appearance. They may remain asymptomatic. They may present for cosmetic reasons, impaired urinary flow, urinary obstruction or dysuria. Most cysts in the pediatric population are smaller than 1 cm. The differential diagnosis includes epidermoid cyst, pilosebaceous cyst, fibroepithelial polyp and juvenile xanthogranuloma. Treatment ranges from follow-up for spontaneous resolution to needle aspiration and surgical excision. In our case, contrary to the literature, the cyst was excised because the cyst was symptomatic, was in the older age group, and enlarged during follow-up. No recurrence was detected in the postoperative follow-up.

Giriş: Parameatal üretral kist, idrar akımının bozulmasına sebep olabilen veya asemptomatik olup kozmetik kaygılara sebep olan nadir bir klinik durumdur. Kendiliğinden geçebilir veya cerrahi eksizyon gerektirir.

Olgu: Oniki yaşındaki hasta 3 yıldır devam eden, dağınık işeme ve üretral mea kenarında kist şikayeti ile başvurdu. Yapılan fizik muayenede, eksternal üretral meatusta 1 cm çapında sferik yapıda, meayı daraltan bir kist saptandı. Hikayesinden travma ve enfeksiyon olmadığı öğrenildi. Kistin boyutunda takibinde büyüme izlendiği için cerrahi eksizyon yapılması planlandı. Kistin eksizyonu sırasında, kist ile üretra arasında herhangi bir bağlantı görülmedi. Patolojisi benign kistik lezyon olarak raporlandı. Takibinde nüks izlenmedi.

Sonuç: Parameatal kist sünnet derisinin glanstan ayrılmasındaki kusurlu süreç nedeniyle oluşur. Bu kistler genellikle doğumda veya erken çocuklukta ortaya çıkar ve üretral meatusun ventral veya lateral kenarında meydana gelebilir. Tanı klinik görünümüne göre konulur. Asemptomatik kalabilirler. Kozmetik nedenlerle, idrar akımının bozulmasıyla, üriner obstrüksiyonla ya da dizüri nedeniyle başvurabilirler. Pediatrik popülasyondaki kistlerin çoğu 1 cm'den küçüktür. Ayırıcı tanı da epidermoid kist, pilosebase kist, fibroepitelyal polip ve juvenil ksantogranüloma yer alır. Tedavisi spontan rezolüsyon açısından takip, iğne aspirasyonu ve cerrahi eksizyona kadar değişir. Bizim olgumuzda literatürün aksine kistin semptomatik olması, büyük yaş grubunda olması ve takiplerinde de büyüme olması nedeniyle kist eksize edildi. Postoperatif takibinde nüks saptanmadı.
