Do Preoperative Leukocyte and Neutrophil Levels Have a Value in Predicting Urethral Separation in Hypospadias Repair?

Objective: Different results can be obtained in the same type of hypospadias performed by the same surgeon and applied the same repair technique. This suggests that there are other unmeasurable factors affecting the results. The aim of our study is to contribute to the literature by determining the value of subclinical inflammation in predicting postoperative urethral separation after hypospadias repair.

Method: We retrospectively analyzed the data of 413 patients who underwent hypospadias repair in our clinic between January 2016 and January 2019. Age at surgery, type of hypospadias, repair technique and complications were recorded. Patients with a single-session hypospadias repair performed by two surgeons were included in the study, while patients with a two-session technique or penoscrotal hypospadias performed by other surgeons were excluded. White blood cell, neutrophil, lymphocyte, platelet and monocyte levels and mean platelet volumes were recorded from the blood test performed one day before the operation. Neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and other parameters were analyzed to determine their relationship with complications after hypospadias surgery.

Results: The mean age of the patients was 43.5 months (6-198 months, median=30 months). There was no difference between patients with and without separation in terms of mean age (p=0.153). Lymphocyte, NLR, and PLR values ​​were statistically higher in patients with separation in urethral repair (p= 0.025, 0.018, 0.07, respectively). Other hematological parameters did not differ between patients. In the Roc analysis, the area under the curve was determined as 79.1%, 19% and 14% for lymphocyte, NLR and PLR, respectively. There was no significant difference between the two surgeons in terms of complication rates and follow-up time between those with and without urethral separation. The mean follow-up period was 46.7 months (12-102 months, median=46 months).

Conclusion: Lymphocyte, NLR, PLR values ​​were statistically significant in predicting urethral separation after hypospadias repair.

Hipospadias onarımınında Preoperatif Lökosit ve Nötrofil Düzeylerinin Üretral Ayrılmayı Öngörmede Bir Değeri Var Mi?

Amaç: Aynı cerrah tarafından yapılan aynı onarım tekniği uygulanan aynı tip hipospadiaslarda farklı sonuçlar alınabilmektedir. Bu durum sonuçları etkileyen, ölçülemeyen başka faktörlerin de olduğunu düşündürmektedir. Çalışmamızın amacı subklinik inflamasyonun, hipospadias onarımı sonrası postoperatif üretral ayrılmayı öngörebilmedeki değerini saptayarak literatüre katkıda bulunmaktır.

Yöntem: Ocak 2016-Ocak 2019 tarihleri ​​arasında kliniğimizde hipospadias onarımı yapılan 413 hastanın verilerini geriye dönük olarak inceledik. Ameliyat yaşı, hipospadias tipi, onarım tekniği, komplikasyonlar kaydedildi. İki cerrah tarafından tek seanslı hipospadias onarımı yapılan hastalar çalışmaya dahil edilirken, diğer cerrahlar tarafından gerçekleştirilen iki seanslı teknik veya penoskrotal hipospadiası olan hastalar çalışma dışı bırakıldı. Ameliyattan bir gün önce yapılan kan testinden beyaz küre, nötrofil, lenfosit, trombosit ve monosit seviyeleri, ortalama trombosit hacimleri kaydedildi. Nötrofil-lenfosit oranı (NLR), trombosit-lenfosit oranı (PLR) ve diğer parametreler, hipospadias cerrahisi sonrası komplikasyonlarla ilişkisini belirlemek için analiz edildi.

Bulgular: Hastaların yaş ortalaması 43.5 ay (6-198 ay, median=30 ay) idi. Yaş ortalaması açısından ayrılma olan ve olmayan hastalar arasında fark yoktu (p=0.153). Üretral onarımda ayrılma görülen hastalarda lenfosit, NLR, PLR değerleri istatistiksel olarak daha yüksekti (sırasıyla p= 0.025, 0.018, 0.07). Diğer hematolojik parametreler hastalar arasında farklılık göstermedi. Yapılan Roc analizinde eğri altında kalan alan lenfosit , NLR ve PLR için sırasıyla % 79.1, %19, %14 saptandı. Üretral ayrılma olan ve olmayan arasında iki cerrahın komplikasyon oranı açısından ve takip süresi açısından anlamlı fark yoktu. Ortalama takip süresi 46.7 ay (12-102 ay, median=46 ay) idi.

Sonuç: Hipospadias onarımı sonrası üretral ayrılmayı öngörmede lenfosit, NLR, PLR değerleri istatistiksel olarak anlamlıydı.
