Comparison of manual detorsion results with surgical findings in a pediatric patient with tes-ticular torsion

Introduction: Emergency intervention in testicular torsion is very important to prevent organ loss. Although manual detorsion of the torsioned testis before surgery is a method used in adult urology, it is rarely used in pediatric patients. In this article, it is aimed to discuss the patients who underwent manual detorsion (MD) in our clinic and their surgical findings.

Material and Methods: 165 patients who applied to our clinic due to testicular torsion between 2018 and 2022 were retrospectively analyzed. 3 newborn patients, 32 patients who did not undergo manual detorsion, and 7 patients with insufficient data were excluded from the study. 123 patients were analysed.

Results: The mean age was 14.4 years. The mean duration of pain was 17.9 hours, and the median was 7 hours (1-120 hours). The mean time to manual detorsion after entering the emergency room was 25.2 minutes. The mean degree of torsion was 488 degrees (180-1440 degrees). There were 90 patients whose pain was completely relieved after manual detorsion. There were 82 patients whose blood supply returned in the Doppler USG performed after MD. There was no one whose pain did not go away among those whose bleeding returned. These 82 patients underwent elective or semi-elective surgery. Patients whose pain did not completely resolve or whose testicular blood flow did not return completely underwent emergency surgery.

Surgical data of 121 patients were obtained. During surgery, residual torsion was found in 20 patients, and complete detorsion of the testis was observed in 101 patients. It was observed that complete detorsion was achieved in all patients who underwent semi-elective or elective surgery. Twenty of 39 patients who were operated on urgently had residual torsion.

Conclusion: Complete relief of pain and complete return of testicular blood supply after manual detorsion are reliable indicators of complete detorsion of the testis. MD can also be safely applied in childhood.

Testis torsiyonu ile gelen çocuk hastada manuel detorsiyon sonuçlarının cerrahi bulgular ile karşılaştırılması

Giriş: Testis torsiyonunda acil müdahale, organ kaybını engellemek açısından çok önemlidir. Cerrahi öncesi torsiyone testise, manuel detorsiyon yapılması erişkin ürolojide uygulanan bir yöntem olmakla beraber pediatrik hastada nadiren uygulanmaktadır. Bu yazımızda kliniğimizde manuel detrosiyon (MD) yapılan hastalar ve onların cerrahi bulgularının tartışılması amaçlanmaktadır.

Method: 2018-2022 yılları arasında testis torsiyonu nedeniyle kliniğimize başvuran 165 hasta geriye dönük olarak incelendi. 3 hasta yenidoğan olduğu için, 32 hastaya manuel detrosiyon uygulanmadığı için, 7 hastanın verilerinde eksiklik olduğu için çalışma dışı bırakıldı. 123 hasta analize alındı.

Bulgular: Yaş ortalaması 14,4 yıl idi. Ağrı süresi ortalaması 17,9 saat, ortancası 7 saat (1-120 saat) idi. Acil servise girdikten sonra manuel detorsiyona kadar geçen süre ortalama 25,2 dk idi. Torsiyon derecesi ortalama 488 derece (180-1440 derece) idi. Manuel detrosiyon sonrası ağrısı tamamen geçen 90 hasta vardı. MD sonrası yapılan doppler USG’de kanlanma geri gelen 82 hasta vardı. Kanlanma geri gelenler arasında ağrısı geçmeyen olmadı. Bu 82 hasta elektif veya semielektif ameliyat edildi. Ağrısı tam olarak geçmeyen veya testis kanlanması tam olarak geri gelmeyen hastalar acil ameliyat edildi.

121 hastanın cerrahi verisine ulaşıldı. Cerrahi sırasında 20 hastada rezidü torsiyon bulundu, 101 hastada testisin tam detorsiyone edilmiş olduğu görüldü. Semielektif veya elektif ameliyat edilen hastaların hepsinde tam detorsiyon sağlanmış olduğu görüldü. Acilen ameliyat edilen 39 hastanın 20’sinde rezidü torsiyon vardı.

Sonuç: Manuel detorsiyon sonrası ağrının tam geçmesi ve testis kanlanmasının tam olarak geri dönmesi testisin tam detorsiyone edildiğinin güvenilir bir göstergesidir. MD çocukluk çağında da güvenle uygulanabilir.
