COMPARISON OF GENERAL VS. LOCAL ANESTHESIA PROTOCOLS FOR JJ STENT REMOVAL IN CHILDREN OVER 12 YEARS OF AGE

Objective: JJ stent removal in children is typically performed under general anesthesia due to cooperation difficulties and high anxiety levels. Since 2024, our clinic has implemented an "awake flexible cystoscopy" protocol for patients over 12 years of age who provide consent after detailed counseling. This study aims to compare local and general anesthesia protocols regarding hospital stay duration and costs, and to evaluate patient satisfaction in the local anesthesia group.
Methods: Patients over 12 years of age undergoing JJ stent removal were analyzed retrospectively by dividing them into general and local anesthesia groups. The local anesthesia protocol included oral antibiotic prophylaxis without intravenous access. In the lithotomy position, after intraurethral lidocaine gel instillation, a 15.5Fr flexible cystoscope was used. Independent Samples T-Test was used to compare hospital stay duration, while the Mann-Whitney U-test was used for cost comparison. Treatment satisfaction in the local anesthesia group was measured, and pain levels were evaluated using a 10-point numerical scale.
Results: The local anesthesia (n=16) group had a significantly shorter hospital stay compared to the general anesthesia (n=32) group (3.6±2.0hours vs. 9.0±1.0hours; p<0.001). Hospital costs for general anesthesia were found to be approximately 3 times higher than those for local anesthesia (p<0.001). The median pain level in the local group was 3/10. Among 11 patients who previously had the procedure under general anesthesia and had local anesthesia this time, 10 stated they would prefer local anesthesia again.
Conclusion: Awake JJ stent removal with a flexible cystoscope in children over 12, reduces hospital stay by 2.5-fold, and decreases costs by 3-fold. It ensures high patient satisfaction when performed on appropriately selected patients. Future plans include performing this procedure with parent accompaniment, and conducting a prospective comparative study.
12 YAŞINDAN BÜYÜK ÇOCUKLARDA JJ STENT ÇEKİLMESİ İÇİN GENEL/LOKAL ANESTEZİ PROTOKOLLERİNİN KARŞILAŞTIRILMASI

Amaç: JJ stent çekilmesi işlemi, çocuklarda kooperasyon güçlüğü ve yüksek anksiyete düzeyi nedeniyle genelde anestezi (LMA veya maske anestezisi) altında gerçekleştirilmektedir. Kliniğimizde 2024 yılından itibaren, 12 yaşından büyük hastalarda, işlem detaylıca anlatılıp eğer onay verirlerse, süreci basitleştirmek adına "uyanık fleksible sistoskopi" protokolü uygulanmaya başlanmıştır. Bu çalışmada, lokal ve genel anestezi protokollerinin hastanede kalış süresi ve maliyet açısından karşılaştırılması; ayrıca lokal anestezi grubundaki hasta memnuniyetinin değerlendirilmesi amaçlanmıştır.
Yöntem: 12 yaşından büyük JJ stent çekilen hastalar genel anestezi veya lokal anestezi gruplarına ayrılarak retrospektif olarak incelendi. Lokal anestezi protokolünde; işlem öncesi oral antibiyotik profilaksisi uygulandı, damar yolu açılmadı. Litotomi pozisyonunda üretraya lidokainli jel sıkılarak 2 dakika beklendi ve 15,5Fr fleksible sistoskop kullanılarak JJ stent çekildi. İşlem sonrası ebeveyn refakatinde 10 dakika gözlem sonrası hastalar taburcu edildi. Gruplar arası yatış süresi karşılaştırmasında bağımsız örneklem T-testi, maliyet karşılaştırmasında ise Mann-Whitney U-testi kullanıldı. Lokal anestezi grubunun işlem memnuniyeti ölçüldü, ağrı düzeyleri 10 noktalı sayısal ağrı skalası ile değerlendirildi.
Bulgular: Hastanede kalış süresi, lokal anestezi grubunda (n=16, 3.6±2.0saat), genel anestezi grubuna (n=32, 9.0±1.0saat) göre anlamlı derecede daha kısadır (p<0.001). Hastane maliyetleri karşılaştırıldığında; genel anestezi maliyeti, lokal anestezi maliyetinin yaklaşık 3 katı olarak saptanmıştır (p<0.001). Lokal gruptaki hastaların medyan ağrı düzeyi 3/10 olarak bulunmuştur. Daha önce genel anestezi ile işlem tecrübesi olan ve bu defa lokal anesteziyle işlem yapılan 11 hastanın 10’u bir daha olursa yine lokal anesteziyi tercih edeceğini belirtmiştir.
Sonuç: 12 yaşından büyük çocuklarda fleksible sistoskop eşliğinde uyanık JJ stent çekilmesi; genel anestezi risklerini ortadan kaldıran, hastanede kalış süresini 2.5 kat, maliyetleri ise 3 kat düşüren, doğru hastalar seçilerek uygulandığında yüksek hasta memnuniyeti sağlayan bir yöntemdir. Gelecekte bu işlemin günübirlik ameliyathanede ebeveyn refakatinde yapılması, hastaların süreci daha iyi kavraması için eğitici videolar hazırlanması için hazırlıklar yapılmakta olup, bir prospektif karşılaştırmalı çalışma planlanmaktadır.
