Experience with Kasai portoenterostomy for the treatment of biliary atresia in a reference center in Colombia

Purpose: Describe the demographic characteristics and native liver survival in patients taken to Kasai portoenterostomy for biliary atresia (BA) in a reference center in Colombia between 2016 and 2024.
Methods: Retrospective cohort study. Data taken from clinical records. A descriptive analysis was made for demographic characteristics using measures of central tendency. For the survival analysis Kaplan Meier curves were used.
Results:
Forty five patients underwent Kasai portoenterostomy (KP) in the time evaluated. Sixty two percent of the patients were female, with a mean age at the KP of 85 days, with 40% of our patients taken to the procedure with more than 90 days of age. Isolated BA was found in 53% of cases, cytomegalovirus (CMV) infection was documented in 35% of our patients, 9% had a cystic BA and only 2.2% had a syndromic presentation. The anatomical classification found most often was type III BA (91 % of cases). Bilirubin clearance was achieved in 35% of patients by 6 months post op, with a native liver survival of 33% for the 8 year follow up.
Conclusions:
Demographic and survival data in patients with BA are scarce in our region; to our knowledge, this is the biggest cohort of patients documented in Colombia. Our population is taken into surgery later in comparison to what's described by other groups worldwide, additionally, we found a higher incidence of CMV infection, which could explain why our children have a lower native liver survival when compared with higher income country´s cohorts, nonetheless, it is comparable to what is reported as general survival for BA worldwide.
