Management of adnexal masses in children and adolescents: a retrospective analysis of a 25-year experience

Purpose:

Adnexal masses in children and adolescents cover a variety of ovarian and paraovarian lesions. A broad range of masses, from benign functional cysts to teratomas, and adnexal torsion require, prompt clinical assessment and diagnostic workup.

Method:

In this retrospective study, 69 patients (neonate-18 years) who underwent surgery due to a wide range of adnexal masses between 1999 and 2024 were analyzed. Differentiation based on important evidences from clinical assessment, diagnostic workup, choice of surgical approach, and histopathological results were investigated.

Results:

Surgical abnormalities of the adnexa included cystic teratomas in 19 cases (the largest 22x21x8 cm), torsion in 15 (oophorectomy only in 6), simple cysts in 21 (the largest 18x15 cm), antenatally diagnosed complex cysts in 5 (ovarian autoamputation), ruptured hemorrhagic cyst in 4, and incidental cysts in 5 (the largest 7x5 cm). A laparoscopic approach was feasible in 29 cases. Large tumoral masses were removed through a Pfannenstiel incision following laparoscopic instrumentation. Histopathologic evaluation identified the resected solid masses as benign teratomas, except for one with yolk sac tumor.
