Surgical Approach to Lung Hydatid Cysts in Children

Aim:Literature data on pulmonary hydatid cysts in children are insufficient. The aim of this study was to determine the treatment options for pulmonary hydatid cysts in children according to the size, location of the cyst and the presentation of the patients.
Methods:Patients 0-18 years of age treated for pulmonary hydatid cyst between 2004-2022 in our clinic were included. Demographic characteristics, treatment modalities, complications and follow-up were evaluated. Data were compared by grouping the patients according to the size of the hydatid cyst and those with ruptured and intact cysts at the time of presentation. ROC curve was created to determine the need for operation according to hydatid cyst size. P<0.05 was considered statistically significant.
Results:A total of 29 female and 43 male patients with a mean age of 8.9±3.4years were included in the study. Hydatid cysts were localized in 64% right lung, 29% left lung and 7% bilaterally. The size of hydatid cysts in the lung ranged between 2.5-18cm and 15 (21.7%) had giant cysts (>10cm). Half of the patients had rupture into the bronchial system or pleural space at presentation. 46 patients (64%) were operated. Of the hydatid cysts with bronchial rupture, 52.4% were in the left lung(p=0.012), their size was smaller than other hydatid cysts(p=0.021) and only 4 (19%) required surgery(p<0.0001). In the ROC curve created to determine the need for operation, the cut-off point was found to be 6.1cm. Patients with cysts that ruptured into the bronchial system and whose cyst contents were emptied by coughing were started on antihelminthic treatment and followed up.
Conclusion:Surgical treatment should be planned without delay for pulmonary hydatid cysts exceeding 6cm in size. Bronchial rupture and coughing up the cyst contents, only medical treatment can be applied.
