Rectourethral fistula is one of the most common forms of anorectal malformation (ARM) in male infants. The standard management involves a staged approach, including initial neonatal colostomy, definitive repair via posterior sagittal anorectoplasty (PSARP), and delayed stoma closure. Despite adherence to this established protocol, complications may occur at any stage, ranging from minor issues to those requiring multiple reoperations and multidisciplinary intervention. This 5 years retrospective study aims to highlight the spectrum of complications, the complexity of management, and the extended clinical journey often faced in such cases, underscoring the need for vigilance, adaptability, and collaborative care in pediatric surgical practice in resource limited lower-middle income country, Nepal.
