Behavioral Approach of the Parents and Surgeons in Nuck Canal Cysts: 15 Years of Experience

Aim:Nuck canal cyst (NCC) is a-rare-condition that can be confused with other pathologies that cause swelling in the-inguinal-region. This-study aimed to evaluate the behavioral-approach of the-family and surgeon in operated-children-diagnosed with NCC.
Method:Hospital-records of children with confirmed NNS’s who underwent surgery for inguinal-cysts/mass in our department between January 2008-2023 were retrospectively reviewed. Demographic-characteristics, complaints, history, examination and surgical findings were evaluated.
Results:In 15-girl diagnosed with NCCs in 15-year, the median-age was 37 months(2months-110months), 8(53%) were preterm, 6(40%) were term, and the gestational-age wasn’t recorded in one. Three(20%) of had noticed the swelling, five(33%) within a-week, and three(20%) after a-week. Seven(46%) had applied to the emergency-department, and eight(53%) had made an-appointment.
It was detected incidentally in 2(13%), one with a-complaint of umbilical-swelling and the-other in the-early control of the-right-inguinal-hernia-operation. The-history of two wasn’t recorded. It could be detected in 3(20%) during the-examination, but not in the others. In 2 of the 10-case(67%) with a-preliminary-diagnosis-of-inguinal hernia, the-swelling was bilateral; in 5 of them, the preliminary diagnosis was NCC.
In 3-case that applied to the-emergency-room, the-swelling was noticed on the-same-day; they were operated with the-preliminary-diagnoses of right-incarcerated-inguinal-hernia, left-reducible-sliding-over-hernia, and right-incarcerated-sliding-over-hernia, respectively. Two(13%) of were operated electively, and 13(87%) were operated urgently. In the operation, NCC was detected on the-right in 12(80%) and on the-Left in 3(20%). In 14, NCCs were excised together with high-ligation of Patent-Processus-Vaginalis;external-ring was closed in-4, and it was narrowed in-1. Their histopathologies were reported as NCC(n:12), fibroadipose-tissue (n: 1),fibroadiposis-muscular-tissue(n: 1) and fibrovascular tissue(n:1).No perioperative and early/late postoperative complications occurred; no recurrence was observed.
Conclusions:It was determined that parents applied urgently/within-the-first-week when the-swelling was first-noticed, and surgeons operated urgently with-the-prediagnosis of incarcerated inguinal-hernia or sliding over-hernia due to un-reductable swelling.It was noteworty that 53% of were preterm and 80% were on the right-side.
