Does the duration of symptoms affect the time to full enteral feeding after pyloromyotomy for infantile hypertrophic pyloric stenosis?

Introduction:

Infantile hypertrophic pyloric stenosis (IHPS) is a common disease. The majority of research on the time to full enteral feeding (TFEF) following surgery focuses on the surgical technique or postoperative feeding technique. Less is known about whether patient characteristics or symptoms can affect the outcome, especially TFEF after pyloromyotomy. The aim of our study was to evaluate if the duration of preoperative vomiting had an impact on TFEF.

Methods

A retrospective chart review of patients who underwent pyloromyotomy at Pediatric Surgery Department "A" between January 2013 and December 2022. Parameters in demographics and in the preoperative and postoperative period were evaluated against TFEF.

Results:

Thirty-seven patients were identified. At presentation, the median range for duration of symptoms was 10 days (3-30 days), and the median time to surgery was 2 days (1-7 days) after admission. Seventeen patients (45.94%) were vomiting in the first day after surgery. Median TFEF was 72 hours after surgery. TFEF increased with the duration of symptoms. Hence, for every day of symptoms, the time for being fully fed increased by one hour.

Conclusion:

In our study, duration of vomiting in hypertrophic pyloric stenosis seems to have a significant impact on the postoperative time to full enteral feeding
