Introduction: This case highlights the diagnostic and therapeutic challenges in managing a pediatric patient with jejunal duplication and ectopic gastric mucosa, a rare congenital anomaly. Nonspecific initial symptoms and imaging mimics of Meckel’s diverticulum underscore the need for comprehensive evaluation in such cases.

Case Presentation: A pediatric patient presented with intermittent abdominal pain, melena, and anemia, later developing severe bleeding. Imaging suggested ectopic gastric mucosa, but laparoscopy revealed jejunal duplication extending into the duodenum, associated with intestinal malrotation. Resection of 40 cm of duplicated intestine and anastomosis were performed, with pathology confirming ectopic gastric mucosa and ulceration. Postoperatively, the patient recovered well, resuming oral diet within a week.

Conclusion: This case emphasizes the importance of integrating clinical, imaging, and surgical findings for accurate diagnosis of rare gastrointestinal malformations. Careful surgical planning to preserve vital structures and long-term follow-up for potential complications (e.g., anastomotic stricture, recurrent bleeding) are critical for optimal outcomes.
