Disorders of Sex Development (DSD): The scenario of DSD clinic- department of Pediatric surgery, Bangladesh Medical University (BMU), Bangladesh
Background: DSD is an emerging issue in developing country like Bangladesh. Most of the DSD patients are not under proper health coverage but people are being aware about this condition from multimedia. Although, some patients knows about their condition but unable to reach their destination because of multifactorial problems. Most of them also deprived from basic rights and privileges of our society. BMU has been initiated DSD clinic for management in Bangladesh.
Objective: To classify DSD and assign their gender for further management
Materials and methods: An observational cross-sectional prospective study was being carried out in DSD clinic, Department of Paediatric Surgery, BMU from Nov. 2021 to Nov. 2023.Total 120 patients attended to our DSD Clinic. Classified (Chicago Consensus-2006) and Gender assignedby their phenotypic, genotypic and hormonal status and managed by our protocol.
Results: Total 120 patients of different types of DSD have been visited. 40%of them present at late (pre-pubertal or pubertal) age. 56 (46%) patients were46 XY DSD, 40(33%) patients were 46 XX DSD, 9(8%)patients weresex chromosome DSD and 15(13%) patients were multiple malformation syndrome.Management protocols include proper sex assignment, counseling, hormone therapy, surgery and follow up. Only out of 120 patients, 20 (16.6%) patients got opportunity of their surgical correction but rests of them are in long queue for operation.
Discussion: DSD a generic definition encompassing any problem noted at birth where the genitalia are atypical in relation to the chromosomes, gonads or anatomical sex [1].An experienced multidisciplinary team that uses a shared decision-making approach with a medical, surgical, ethical, psychological and human rights framework is required to maximise longterm positive outcomes for people born with DSD.
Conclusion: People are becoming aware of DSD and seektheir management.However, social awareness and management facility for DSD is crucial but inadequate.
