Purpose:

In Bangladesh, female medical students have traditionally pursued careers in obstetrics and gynaecology or paediatric medicine—fields perceived as “safe” and socially acceptable choices for women. These specialties are often associated with quicker financial return and perceived as more compatible with family life. In contrast, surgical specialties—particularly paediatric surgery—are considered physically demanding, time-consuming, and less financially rewarding in the short term, which discourages many female doctors from choosing the field. However, a gradual shift is occurring, with more female doctors now venturing into paediatric surgery. This study aims to explore the social, structural and institutional barriers that limit female participation in paediatric surgery in Bangladesh, and to highlight emerging enablers that are fostering change.

Methods:
A narrative review was conducted based on data collected through structured interviews, observation, and analysis of trends in career choices among female doctors over the past decade. Reflections from academic and clinical institutions were also reviewed to contextualize the findings.

Results:

Paediatric surgery is seen as emotionally and physically taxing, with long training and limited private practice opportunities. However, encouraging developments include mentorship from senior surgeons, supportive institutional environments, the visibility of successful female paediatric surgeons, and growing awareness around gender equity. A new generation of female doctors is increasingly motivated to challenge stereotypes and follow their true professional interests.

Conclusion:

Although still underrepresented, female doctors in Bangladesh are gradually entering paediatric surgery. Addressing social barriers while strengthening support systems such as mentorship and institutional encouragement is vital to sustaining this positive trend and ensuring a more inclusive future in surgical specialties.
