Aim: To evaluate the predictive value of serum amiloid A (SAA) in childhood abdominal pain compared with sedimentation and total leukocyte count.

Patients and Methods: Between January 2010 and January 2011 the patients, who admitted with acute abdominal signs were included in the study and grouped into 3. Group 1 contained the patients with acute appendicitis and taken to operation. Group 2 contained the patients who admitted with acute abdominal signs and observed non-operatively. Group 3 contained the patients admitted to the emergency service with the syptoms other than related to abdominal pain. Blood samples for evaluation of SAA, sedimentation and total leukocyte count were collected on admission from all patients. In Group 1, the blood samples were collected soon after the operation and on the post-operative 24th hour. In Group 2, the same tests were made similtaneously after and on the 24th hour of admission. The statistical analyes were performed with One-way ANOVA and post-hoc Tukey tests. The data were presented as mean ± standard deviation. P was significant at < 0.05.

Results: Each group contained 23 patients. The sex and age distrubution of the groups were similar. Evaluation of SAA values revealed a significant difference between the groups with the values; Group: 1 935219.0±169387.3 mg/L, Grup 2: 235691.3±133271.4 mg/L and Grup C: 308615.7±280936.7 mg/L (p<0.000). Statistical comparison of total leukocyte count in Group 1 (16223.0±5072.7x10^3/uL), Group 2 (13333.0±7900.4 x10^3/uL) and Group 3 (13304.3±4448.6 x10^3/uL) were comparable (p=0.17). The comparison of sedimentation in Group 1 (17.69±11.68 mm/h), Group 2 (16.08±12.58 mm/h) and Group 3 (10.95±12.36 mm/h) were also comparable (p=0.15).

Discussion: We believe that blood SAA measurement is more predictive than total leukocyte count and sedimentation in presumption of surgery in childhood abdominal pain. These results need to be evaluated with further studies.

Amaç: Bu çalışma, çocukluk çağı karın ağrılarının cerrahi endikasyon belirlenmesinde kan serum amiloid A (SAA) düzeyi ölçümünün güvenirliğinin beyaz küre (BK) ve sedimentasyon değerleri ile kıyaslanmasını amaçlamaktadır.

Gereç ve Yöntem: Ocak 2010-Ocak 2011 arasındaakut karın bulgularıyla kliniğimize başvuran 0–17 yaş arası hastalar çalışmaya alındı. Hastalar üç gruba ayrıldı. Grup 1, akut apandisit ön tanısı ile operasyona alınan, Grup 2 akut karın ön tanısı ile servise yatırılıp tıbbi takip ile ağrısı gerileyen ve Grup K şikayetine bakılmaksızın acil servise başvuran ve tanısına sonradan bakılan hastaları kapsamaktaydı. Tüm hastalardan başvuru anında, Grup 2’de yatış sonrası 24. saatte, Grup 1’ de ise operasyon sonrası erken dönem ve 24. saatte olmak üzere SAA, sedimentasyon ve BK çalışıldı. Operasyon sonrasında hastaların tüm tetkikleri ve patoloji sonuçları ile kesin tanıları incelendi. Sonuçlar ortalama ± standart sapma olarak ifade edildi ve One-way ANOVA ve post-hoc Tukey testleri ile değerlendirildi. P < 0,05 anlamlı olarak kabul edildi.

Bulgular: Her çalışma grubu 23 hasta içermekteydi. Tüm gruplar yaş ve cinsiyet dağılımı açısından benzerdi. Grup 1’ de ortalama SAA düzeyi 935219,0±169387,3 mg/L, Grup 2’ de 235691,3±133271,4 mg/L ve Grup K’ da 308615,7±280936,7 mg/L idi (p<0,000). Grup 1’ de ortalama BK sayısı 16223,0±5072,7 x10^3/uL, Grup 2’ de 13333,0±7900,4 x10^3/uL ve Grup K’da 13304,3±4448,6 idi (p=0,17). Ortalama sedimentasyon değeri Grup 1’ de 17,69±11,68 mm/saat, Grup 2’ de 16,08±12,58 mm/saat ve Grup K’ da 10,95±12,36 idi (p=0,15).

Tartışma: SerumSAA seviyesinin, çocukluk çağında acil cerrahi gerektiren karın ağrısını belirlemede sedimentasyon ve BK’ ye göre daha değerli bir belirteç olduğunu düşünmekteyiz ancak bulguların geniş hasta gruplarında değerlendirilmesi yararlı olacaktır.
