Intestinal gangrene due to mesenteric thromboembolism caused by thrombophilia mutations

           Background: Altered balance between the fibrinolytic and procoagulant systems might significantly contribute to the pathophysiology of thrombus formation. Thrombophiliatic mutations always cause a tendency for venous thrombosis.The prevalence of venous thromboembolism in the childhood is 1/100.000. 

 

          Case report: A 15-year old boy applied to emergency clinic with progressively increasing abdominal pain for four days. Associated symptoms were bilious vomiting, and non-passage of stools. There was not any special information in his history. Surgery was advised.

             On examination, body temperature was 37.8ºC,blood pressure was 80/40 mmHg, pulse was 106/min.,  WBC was 10.200/ml, Hb was 14.4 g/dL and  CRP was 386 mg/dl . The abdomen was distended, tender and rigid with no bowel sounds. A provisional diagnosis of with acute abdomen was made, and he was explored. Through a midline incision, the abdomen was explored and a massive gangrenous bowel was observed involving the distal ileum, cecum, ascending colon and proximal threefourths of the transverse colon. Resection of necrotic intestinal segment following ileocolostomy was performed in patient. Resection specimens were reported as transmural infarct by pathology.

           Conclusion: In the genetic analysis, heterozygous (Factor V H1299R (R2), β- Fibrinogen-455 G-A, PAI-1 4G-5G 4G/4G) and homozygous (GPIIIa L33P (HPA-1) platelet, ACE D/D genes) mutations were found. Warfarin therapy was suggested for prophylaxis by hematology. We concluded that genetic analysis is very important and could be helpful to understand the ethiology of embolism seen in childhood.

Trombofili Mutasyonuna Bağlı Mezenterik Emboli Sonucu İntestinal Gangren

            Amaç: Tromboz damar içinde kan elemanlarından oluşan anormal bir kitledir. Prokoagülan, antikoagülan ve fibrinolitik faktörler arasındaki hassas dengelerin bozukluğu sonucunda oluşur. Kalıtsal trombofililerin tamamı venöz tromboza eğilim yaratır. Çocukluk çağında tromboz sıklığı ortalama 1/ 100.000’dir.

               Olgu Sunumu: On beş yaşındaki erkek hasta dört gündür devam eden karın ağrısı yakınması ile acil servisimize başvurdu. Ağrıya, karın şişliği ve gaita çıkaramama, safralı kusma yakınmaları da eşlik ediyordu. Öz ve soy geçmişinde bir özellik yoktu.

               Muayenede, tansiyon 80/40 mmHg, nabız 106/dakika ve vücut ısısı 37,8ºC idi. Beyaz küre: 10.200/ml, Hb:14,4 g/dL CRP: 386 mg/dl idi. Karın distandü, hassasiyet, defans mevcuttu ve barsak sesleri alınamıyordu. İleus, akut batın ön tanılarıyla operasyona karar verildi. Orta hat insizyonla yapılan eksplorasyonda terminal ileum, çekum ve transvers kolonun ¾’ünün dolaşımının bozulmuş, ileri derecede dilate olduğu gözlendi. Dolaşımı bozulmuş olan barsak ansları rezeke edilerek, ileokolostomi yapıldı.

                Rezeksiyon materyallerinin patolojik incelemesinde; transmural infarktüs olarak raporlandı. Hastada tromboza neden olabilecek olaylar araştırıldığında herhangi bir risk faktörü saptanamaması üzerine tromboza neden olabilecek gen paneli çalışıldı. Olgumuzun gen panelinde; Faktör V H1299R (R2), β-Fibrinojen -455 G-A heterozigot, PAI-1 4G-5G 4G/4G, GPIIIa L33P (HPA-1), ACE D/D, homozigot mutasyonlar saptandı. Yaşam boyu kullanmak üzere hematoloji tarafından profilaktik olarak Warfarin başlandı.

                Sonuç: Çocuk yaş grubundaki emboli olaylarının etiyolojisinin aydınlatılmasında trombofili gen paneli çalışmasının önemli olabileceğini düşünmekteyiz.
