SWALLOWING EVALUATION RESULTS OF A PATIENT WITH FACIOSCAPULOHUMERAL MUSCULAR DYSTROPHY

Purpose :

Facioscapulohumeral Muscular Dystrophy (FSHD) is a type of muscular dystrophy that affects the muscles over time, characterized by the progressive weakness of the facial and shoulder muscles. The prevalence of FSHD is 1: 20000 and indicating a genetic transition. There are limited number of studies about swallowing disorders in FSHD.The purpose of this study is to evaluate the swallowing function of a FSHD diagnosed patient who applied to Gazi University Swallowing Disorders Unit.

Materials and Methods:

İ.A, a 47-year-old male patient had history of major depression, HT and cervical-lomber disc herniation. The patient had been followed up by the psychiatry service in consequence of recently increased depressive complaints. There is also a history of “foot drop” effecting the lower right extremity of the patient for the last 6-7 months. On account of the complaints including difficulty in walking and pain, Electromyography(EMG) had been performed and active myopathy was detected. Then, the patient was followed up by neurology with diagnosis of "Facioscapulohumeral Muscular Dystrophy". In recent days, with increased sense of hesitation during swallowing, the patient was referred by Psychiatry Department.

Conclusion:

The patient experiences no pain during swallowing but describes a feeling of hesitation especially during fluid intake. Head control and sitting balance of the patient were good. The patient had no previous history of pulmonary infection and no complaints of weight loss. Voluntary cough and voluntary apnea were observed. While the GAG reflex couldn’t be seen in the patient, weakness of the tongue movements were observed throug oral motor evaluation. The oral hygiene of the patient was good. As a result of FEES; PAS score 1 was found in solid foods, while a slight residue was observed in pyriform sinuses. The PAS score was 5 in liquids and delayed swallowing reflex was observed. As a result, the FOIS score was set at 6.

Discussion:

Lingual and chewing muscles could be affected in individuals with FSHD. The influence of lingual and chewing muscles affect the swallowing function due to the important roles in preparing and transferring of bolus during the oral phase of swallowing. These patients may have delays in triggering the pharyngeal phase of swallowing.

FACİOSCAPULOHUMERAL KAS DİSTROFİLİ BİR OLGUNUN YUTMA DEĞERLENDİRME SONUÇLARI

Amaç:

Facioscapulohumeral Muscular Dystrophy (FSHD) yüz ve omuz kuşağı kaslarının progresif zayıflığı ile karakterize olup zamanla abdominal ve ayak bileği ekstansörlerini de etkileyen bir kas distrofisi tipidir. Prevalansı 1:20000 olup genetik geçiş göstermektedir. FSHD’ de yutma bozuklukları ile ilgili literatürde sınırlı sayıda çalışma mevcuttur. Bu çalışmamızın amacı; Gazi Üniversitesi Yutma Bozuklukları Birimine yönlendirilen FSHD tanılı olgunun yutma fonksiyonunun değerlendirilmesidir.

Gereç ve Yöntem:

İ.A, 47 yaş, erkek hastanın özgeçmişinde; majör depresyon, HT ve servikal-lomber disk herniasyonu mevcuttur. Hastanın son zamanlarda artan depresif şikayetleri sebebiyle Psikiyatri servisinde takibe alınmıştır. Ayrıca son 6-7 aydır sağ alt ekstremiteyi tutan düşük ayak öyküsü mevcuttur. Yürümede zorlanma ve ağrı şikayetleri ile yapılan EMG sonucunda aktif myopati saptanmıştır. Hasta “Facioscapulohumeral Muscular Dystrophy” ön tanısı ile Nöroloji takibine alınmıştır. Son günlerde artan yutkunma esnasında takılma hissi ile, Psikiyatri tarafından Gazi Üniversitesi Yutma Bozuklukları Birimi’ne yönlendirilmiş ve tarafımızca değerlendirilmiştir.

Sonuç:

Hasta, özellikle sıvı alımı sırasında boğazında takılma hissi tariflerken, yutkunma esnasında ağrı yaşamamaktadır. Baş kontrolü ve oturma dengesi iyi olan hastanın 2 yıl süren ve tedavi ile sonuçlanan reflü hikayesi mevcut iken, daha önce geçirilmiş bir akciğer enfeksiyonu öyküsü ve kilo kaybı şikayeti yoktu. Hastada istemli öksürük ve istemli apne vardı. Oral motor değerlendirmede hastada GAG refleksi alınamazken, dil hareketlerinde zayıflık gözlendi. Diş fırçalama alışkanlığı olan hastanın ağız hijyeni iyiydi. Fiberoptik Endoskopik Yutma Değerlendirmesi (FEYD) sonucunda; katı gıdalarda Penetrasyon Aspirasyon Skalası(PAS) skoru 1 bulunurken, piriform sinüslerde hafif göllenme izlenmiştir. Sıvı gıdalarda PAS skoru ise 5 bulunmuş ve yutma refleksinde gecikme gözlenmiştir. Sonuç olarak FOIS skoru 6 olarak belirlenmiştir.

Tartışma:

Facioscapulohumeral kas distrofili bireylerde lingual kaslar ve çiğneme kasları etkilenebilmektedir. Yutmanın oral fazında bolusun uygun kıvama gelmesinde ve transferinde önemli görevleri olan lingual kaslar ve çiğneme kaslarının etkilenmesi yutma fonksiyonunu etkileyebilmektedir. Bu hastalarda özellikle farengeal faza geçişte gecikmeler yaşanabilmektedir.
