Investigating the efficacy of short-term lansoprazole use in the treatment of reflux esophagitis in children

Aim

The aim of study comparing the efficacy and side effects of lansoprazole short and long-term use recommended for use in reflux esophagitis childhood group.

Method

This study was conducted with 148 patients between the ages of 8-18 years who were diagnosed reflux esophagitis with endoscopy between January 2017 and January 2018. Changes in lifestyle habits and diet were recommended for all patient reflux. In addition, the patients were randomly distributed on treatment and all group together with sodium alginate and a single oral dose of lansoprazole; <30 kg 15 mg / gun,> 30 kg 30 mg / gun was used for 4 weeks in group 1 and 8 weeks in group 2. Control endoscopy was performed at the end of the treatment in both groups and the esophagitis classification according to the Los Angeles classification was compared. In addition, recovery was questioned in the initial dispeptic complaints. At the same time, headache, abdominal pain, diarrhea, vomiting and bloating were considered side effects that were not initially present and developed after drug use.

Results

Histopathologically, 9 patients who were diagnosed as eosinophilic esophagitis, 8 patients who did not accept the study and 13 patients who did not complete the treatment were excluded from the study. The remaining 118 patients were randomly distributed and the first group consisted of 58 second group of 60 patients. In the first group, 89.5% improvement was observed at the end of 4th week whereas in the 2nd group this rate was 91.2% at the end of 8th week. The difference was not statistically significant (p> 0.05). In addition, the most common adverse events in both groups were abdominal pain, headache and diarrhea in the second group, 3,4% -3,7% -3,1%, and in the first group these rates were less frequent and 1,6% 2.1% -1.9% and the difference was not statistically significant but was observed less frequently in the first group.

Conclusion

In children with reflux esophagitis due to gastroesophageal disease, the use of lansoprazole for 4 weeks is as effective as the use of lansoprazole for 8 weeks. In addition, side effects are less noticeable in short-term use.

Çocuklarda reflü özofajitin tedavisinde kısa süreli lansoprazol kullanımının etkinliğinin araştırılması

Amaç

Bu çalışma reflü özofajitte çocukluk yaş grubunda kullanımı önerilen lansoprazolun kısa ve uzun süreli kullanımında tedavideki etkinliğini ve yan etkinliğini karşılaştırmak amacı ile yapıldı.

Yöntem

Bu çalışma 2017 Ocak-2018 Ocak ayları arasında endoskopi ile reflü özofajit tanısı konan 8-18 yaş arasındaki toplam 148 hasta ile yapıldı. Histopatoloji sonucunda eozinofilik özofajit tanısı alan hastalar çalışmadan çıkartıldı. Bütün hastalara reflü için yaşam alışkanlıkları değişiklikleri ve diyet önerildi. Ayrıca tedavide hastalar rastgele olarak dağıtılmış olup tüm gruba sodyum aljinat ile birlikte ve oral tek doz lansoprazol ; <30 kg 15 mg/gun, >30 kg 30 mg/gun olacak şekilde 1. Grupta 4 hafta ve 2. Grupta 8 hafta kullanıldı. Her 2 grupta da tedavinin bitiminde kontrol endoskopi yapıldı ve Los Angeles sınıflamasına göre yapılmış olan özofajit sınıflaması karşılaştırıldı. Ayrıca başlangıçta olan dispeptik şikayetlerde düzelme sorgulandı. Aynı zamanda başlangıçta olmayan ve ilaç kullanımı sonrası gelişen baş ağrısı, karın ağrısı, ishal, kusma ve şişkinlik yan etki olarak kabul edildi.

Bulgular

Histopatoloji sonucunda eozinofilik özofajit tanısı alan 9 hasta, çalışmayı kabul etmeyen 8 hasta ve tedaviyi tamamlamayan 13 hasta çalışmadan çıkartıldı. Geriye kalan 118 hasta rastgele olarak dağıtılmış olup 1. Grup 58, 2. Grup 60 hastadan oluştu. Birinci grupta 4. haftanın sonunda % 89,5 oranında iyileşme gözlenirken 2. Grupta 8. Haftanın sonunda bu oran % 91,2 idi. Aradaki fark istatistiksel olarak anlamlı gözlenmedi (p>0.05). Ayrıca her iki gruptada en sık gözlenen yan etkiler karın ağrısı, baş ağrısı ve ishal olup 2. Grupta sırasıyla %3,4-%3,7-%3,1 olup 1. Grupta bu oranlar daha az sıklıkta gözlenmiş olup sırasıyla % 1,6- %2,1-%1,9 idi ve aradaki fark istatistiksel olarak anlamlı olmamakla birlikte 1. Grupta daha az sıklıkta gözlendi.

Sonuç

Gastroözofageal hastalığa bağlı reflü özofajit gelişen çocuklarda tedavide 4 hafta lansoprazol kullanımı 8 hafta lansoprazol kullanımı kadar etkilidir. Ayrıca yan etki kısa süreli kullanımda daha az gözlenmektedir.
