THORACOSCOPIC TREATMENT OF PULMONARY HYDATID CYST IN CHILDREN

Background and Aim:Surgical treatment has remained the standard option in the management of hydatid disease. Fourteen pediatric cases of pulmonary hydatid cyst, treated successfully using the thoracoscopic approach,  were retrospectively reviewed in this study.

Material and Methods:From April 1999 to December 2010, 10 boys (71%) and 4 girls (29%) aged between 6 and 16 years (average 9 years) old have been operated on for hydatid cyst of the lung by a thoracoscopic procedure. All of  the patiens receive oral albendazole 10-12 mg/kg/day soon after diagnosis.  The patients were put in a full lateral decubitus position. Two 5-mm and one 10 mm cannulas were placed in a triangular fashion.  A 5 mm 30o  telescope was introduce through middle port. For noncomplicated cysts the hypertonic saline was injected, and then the endocysts were extracted using a 10 mm endobag. Irrigations, partial decortications and partial excision of the exocyst were performed and the cyst in using suture ligation for air leak. Infected endocysts and partial exocyst were extracted as the same manner. A chest tube was placed through the lower port site.

Results:It was a vomited cyst in 1 case, a pyopneumocyst in 7 cases, and a noncomplicated and univesicular cyst in 6 cases. The patients had uneventful postoperative courses. Median follow up period was 5 years (range 6 months to 10 years). In follow-up period, they were free of symptoms with no recurrence on chest x-ray.

Conclusion:All stages of the surgical treatment of hydatid cyst of the lung in children can be accomplished by thoracoscopy. Thoracoscopic management of pulmonary hydatid cyst is safe, offers the advantages of less pain, rapid recovery, less short and long term morbidity, and good cosmesis. 

ÇOCUKLARDA AKCİĞER KİST HİDATİKLERİNİN TORAKOSKOPİK TEDAVİSİ

Giriş ve Amaç: Kist hidatik tedavisinde cerrahi hala en geçerli yöntemlerden birisidir. Bu çalışmada,torakoskopik yaklaşımla tedavi edilen, akciğer kist hidatikli, 14çocuk hastaretrospektif olarak incelendi.

Materyal ve Metod: Nisan 1999 – Aralık 2010 tarihleri arasında, akciğer kist hidatiği olan ve ortalama yaşları 9 yaş (6–16 yaş), 10’u (%71) erkek, 4’ü (%29) kız toplam 14 hasta, torakoskopik yaklaşımla tedavi edildi. Hastalar, tanı konulduktan sonra, 10–12 mg/kg/gün olacak şekilde oral yoldan albendazol aldılar. Lateral dekubitus pozisyonunda yatırılan hastalarda iki 5 mm, bir adet 10 mm’lik trokar, üçgen oluşturacak şekilde yerleştirildi. Ortadaki trokar teleskop (5 mm, 30o) için kullanıldı. Komplike olmayan kistlere hipertonik tuzlu su enjeksiyonu yapıldıktan sonra germinatif membranlar 10 mm endobag kullanılarak çıkarıldı. Kist duvarı parsiyel olarak eksize edilip parsiyel dekortikasyon yapıldı. Komplike kistlerde germinatif membran ve parsiyel olarak eksize edilen kist duvarı aynı şekilde dışarı çıkarıldı. Hava kaçakları sütüre edildi. İşlem sonunda alttaki trokar giriş yerinden göğüs tüpü yerleştirildi.

Bulgular: Bir hasta germinatif membran ve kist sıvısını ağız yoluyla çıkarmıştı, 7 hastada enfekte tek kist, 6 hastada ise komplike olmayan tek kist mevcuttu. Hastaların hepsi ameliyat sonrası dönemi sorunsuz geçirdi. Ortalama takip süresi 5 yıl (6 ay – 10 yıl) idi. Takiplerinde herhangi bir komplikasyon olmadı, kontrol akciğer grafilerinde de kist görülmedi.                .                                  

Sonuç: Çocuklardaki akciğer kist hidatikleri, tüm evrelerinde, torakoskopik yaklaşımla tedavi edilebilmektedir. Akciğer kist hidatiklerinin tedavisinde torakoskopik girişim, daha az ağrı, çabuk iyileşme, iyi kozmetik görünüm, daha düşük morbidite gibi avantajlara sahip bir yöntemdir.
