Hepatic Hydatid Cyst and Intra-peritoneal Free Hydatid Cyst

Introduction: Hydatid disease is relatively frequent in our country and non-invasive ultrasonic imaging techniques have made possible an earlier diagnosis prior to serious complications. Human hydatid disease usually occurs by infestation with Echinococcus granulosus and less frequently with Echinococcus multiocullaris. Humans receive the disease through enteral exposure and become accidental intermediate hosts. Hydatid disease is an endemic problem in Turkey as well as in sheep-breeding regions of the world. Hydatid disease may be located in any organ of the body. The most frequently involved organ is the liver (50% to 70%), while the lungs are the second most common site (20% to 30%).

 The cyst may be ruptured after trauma or spontaneously as a result of increased intracystic pressure. The most frequent complication is rupture of the cyst, either internally or externally, followed by secondary infection, anaphylactic shock, and liver displacement in decreasing frequency with the same order.

Our Case:A twelve-year old girl fell from the swing 12 days ago. The patient was suspected of acute abdomen and was operated under emergency conditions. A cystic formation white in color and wall thickness approximately 15 cm was observed at the level of umbilicus near the midline. Debridement of the abdominal areas was made with blunt dissections and povidine iodine pads were used.

The patient received post-operative Andazol treatment.          

Currently, the patient is still under follow-up at month 36. Recurrence was not detected at the time of writing this report.

Discussion : Exploration of the abdomen of our patient revealed a giant cystic lesion formation, and this development did not fit the development of hydatid cyst pathophysiology. Under normal conditions, such cysts grow 0.8-1cm per year in 12-year-old children, therefore, reaching of the cyst to a size of 15 cm is not possible. In addition, we know that the cyst formation in tissues lead to the formation of pseudocapsules. But in this case, formation of pseudocapsule was not present. The mass was separated Very easily from the mesentery and intestines, and adhesions were not present and the cystic lesion was removed as a whole. Vesicles were cleared  This case does not conform to the pathophysiology of cyst development. We suggest that the development of this particular cystis an exception or review of the pathophysiology of development. Also agree with the opinion of surgical treatment, in addition to preventing recurrence of Albendazole Treatment.

Hepatik Kist Ve İntraperitoneal Serbest Kist

Giriş:Hydatid hastalığı ülkemizde sık görülen, non invaziv tetkiklerle teşhis edilebilen ve çeşitli komplikasyonlara sebep olabilen bir hastalıktır. İnsanlarda bu hastalık genellikle Echinococcus granulosus infestasyonu sonucu oluşur. Daha az sıklıkla Echinococcus multiocullaris infestasyonu hastalık nedeni olabilmektedir. İnsanlar enteral yolla bu paraziti alır ve ara konakçı durumundadır. Türkiyede özellikle kırsal alanda bu hastalık endemik olarak bulunmaktadır. Hydatit hastalığı birçok organı tutabilir. En çok tutulan organ karaciğerdir(%50-70). Akciğer bu organdan sonra ikinci sıklıkta tutulur(%20-30).

Kist rüptürü travma sonrası veya spontan olabilen hydatid hastalık komplikasyonlarındandır. Bunun nedeni intrakistik basıncın artmasıdır. Komplikasyonları, kistin internal yada eksternal rüptürü, sekonder enfeksiyon, anaflaktik şok, karaciğer ve diğer organlara bası ile oluşabilen komplikasyonlardır.

Olgu: 12 yaşında kız hasta, 12 gün önce salıncaktan düşme hikayesi var. Akut batın bulguları nedeni ile opere edildi. Explorasyonda, yaklaşık 15 cm çapında beyaz renkli kistik kitle göbeğin yanında ve orta hatta, serbest olarak tespit edildi. Ve tamamen çıkarıldı. İntra abdominal veziküller temizlendi. Albendazol tedavisi ile takip edildi. 36 aylık takibinde nüks bulgusu tespit edilmedi.

Tartışma: Bizim tespit ettiğimiz vakada kısa süre içinde batın içinde dev kistik lezyon oluşumu  mevcuttu  ve bu gelişim hidatid kist patofizyolojik gelişimine uymuyordu. Normal şartlarda kist bir yılda 0.8-1 cm büyümektedir. Kistin 12 yaşındaki çocukta 15 cm büyüklüğe ulaşması mümkün değildir. Ayrıca biz biliyoruz ki, kist oluşumunda dokuların oluşturduğu pseudokapül mevcuttur. Ama vakamızda bu oluşum mevcut değildir. Mezenterden ve barsaklardan çok rahat ayrılan, belirgin yapışıklık olmayan kistik lezyon cerrahi olarak tam çıkarılmıştır. Ayrıca bol miktarda küçük vezilüller bulunmaktadır ve hemen hepsi cerrahi olarak temizlenmiştir. Bu olay kistin gelişim patofizyolojisine uymamaktadır. Kist gelişiminin istisnalarının olacağını veya gelişim patofizyolojisinin tekrar gözden geçirilmesi gerektiğini düşünüyoruz. Ayrıca tedavide cerrahi girişime ilaveten Albendazol tedavisinin rekürrensi önlediği görüşüne katılıyoruz.
