SACROCOCCYGEAL TERATOMA? HUMAN TAIL?

Introduction: Human tail is a midline defect which consists of adipose tissue, muscle fibers, vascular tissue and no bone connection. Usually it doesn’t contain bone and cartilage tissue but there is also a report of a patient with four vertebrae in the tail in the literature. Human tail disease is seen rarely. Association of spinal dysraphism, tethered cord, teratoma, meningocele, spina bifida, and lipoma was seen with human tail. It may be the first sign of these pathologies. There are 2 types: pseudo-tail and true tail. Treatment of the human tail disease is total excision of the mass.

 

Case: The patient was 11 years old. He presented with complaints of pain and there was a mass in the sacrococcygeal region. Approximately 6x5 cm in size, it was containing a few masses of hard consistency. With the initial diagnosis of human tail, an magnetic resonance imaging (MRI) was performed. The mass had cystic components and it was not associated with the spinal canal and rectum. Spinal canal and the dorsal nerve roots were normal. AFP and B-HCG was normal. Operation was performed with an eliptical incision around the mass under general anesthesia; the mass was excised totally and by taking a piece of coccyx. Pathological examination revealed a cyst lined with keratinizing squamous epithelium; and skin appendages, cartilage, respiratory epithelium, fat and smooth muscle tissue was observed around the cyst wall. The diagnosis was mature cystic teratoma with these findings. The patient was discharged on the postoperative fifth day. Postoperative early complications weren’t observed. There was no recurrence during the three-month follow-up.

 

 

Discussion: In our case, because of presence of a midline mass and the patient's older age, the mass was thought to be human tail. But the pathology of the mass came out to be sacrococcygeal teratoma. Presence of benign germ cell pathology is surprising at this age.

SAKROKOKSİGEAL TERATOM? HUMAN TAIL?

Giriş: Human tail, içerisinde adipöz doku, kas lifleri ve damar dokusu ihtiva eden, kemik bağlantısı olmayan orta hat lezyonlarına verilen isimdir. Genellikle kemik ve kıkırdak dokusu ihtiva etmemekle birlikte literatürde 4 vertebra içeren bir olgu da mevcuttur. Nadir görülen bir hastalıktır. Spinal disrafizm, gerilmiş omurilik, teratom, meningosel, spina bifida, lipom gibi hastalıklarla birlikteliği görülmüştür. Bu rahatsızlıkların ilk belirtisi olarak da görülebilir. Gerçek ve yalancı olmak üzere 2 tipi vardır. Tedavisi kitlenin total eksizyonudur.

 

Olgu: On bir yaşında olan hasta sakrokoksigeal bölgede kitle ve ağrı şikâyetleriyle kliniğimize başvurdu. Yapılan muayenesinde, sakrokoksigeal bölgede yaklaşık 6x5 cm boyutlarında, içerisinde birkaç adet sert kıvamda ele gelen kitlesi bulunan lezyon gözlendi. Human tail ön tanısıyla MR istendi. Çekilen MR da kistik komponentleri olan, 5x3x4 cm boyutunda kitle izlendi. Kitlenin spinal kanal ve rektumla ilişkisi yoktu. Spinal kanal ve dorsal sinir kökleri normal izlendi. AFP, B-HCG normaldi. Kitle genel anestezi altında, eliptik insizyonla girilerek, total olarak ve koksiksten de bir parça alınarak eksize edildi. Patolojik incelemesinde keratinize çok katlı yassı epitelle döşeli kist cidarı ve kist cidarında deri ekleri, kıkırdak, respiratuar epitel, yağ ve düz kas dokusu izlendi. Bu bulgularla olgu matür kistik teratom olarak değerlendirildi. Hasta, postoperatif 5. günde taburcu edildi. Postoperatif dönemde erken komplikasyon izlenmedi. 3 aylık izlem süresince nüks görülmedi.

 

 

Tartışma: Olgumuzda hasta kitlenin lokalizasyonu ve yaşının büyük olması nedeniyle ilk planda kitlenin human tail olabileceği düşünüldü. Kitlenin patolojisinin sakrokoksigeal teratomla uyumlu olması ve yaş itibariyle malignansi gelişmemiş bir germ hücreli tümörle karşılaşmış olmak şaşırtıcıdır.
