EXPERIENCES ON PERCUTANEOUS ENDOSCOPİC GASTROSTOMY

 

Purpose:To present our experiences on percutanous endoscopic gastrostomy.

Cases and Method: 28 cases of percutanous endoscopic gastrostomy handled in our clinic from 1995-2010 have been analyzed with backtracking. Cases have been studied according to age, gender, upper GIS passage graphies, post-operative complications, feeding times and hospital stay durations.

Cases:16 of the cases are female, 12 are male. Average age is 6.4 (in range 4 months – 15 years). In all of the cases, the major indication to PEG was inability to feed orally and failure to thrive. Twenty seven cases were being monitored due to neurological illness. There was eosophageal narrowing due to corrosive substance intake in one case. The weight average of the cases was under 10 percent. GER was detected in 4 pre-operational barium passage graphies. Gastrostomy had to be opened in one case due to technical problems experienced during installation of PEG. In post-operational period, one case had to be revised because PEG had shifted under the skin. In three cases PEG was replaced due to clogging. In none of the cases GER was observed after the operation. Weight gain has been observed in all cases during a monitoring period of approximately 6 months.

Conclusion:PEG is a straightforward, effective and reliable method to enable long-term intestinal feeding for especially neurologically problematic cases. 

PERKÜTAN ENDOSKOPİK GASTROSTOMİ DENEYİMİ

 

Amaç: Perkütan endoskopik gastrostomi(PEG) deneyimlerimizi aktarmaktır.

Olgular ve Yöntem: Kliniğimizde 1995-2010 yılları arasında  perkütan endoskopik gastrostomi yapılan 28 olgu geriye dönük olarak incelendi. Olgular yaş, cins, nörolojik durum, üst GİS pasaj grafileri, postoperatif komplikasyon, beslenme zamanları ve hastanede kalış süresine göre değerlendirildi.

Bulgular: Olguların 16 ‘sı kız, 12’si erkek idi. Yaş ortalaması 6.4 yaş ( 4 ay-15 yaş) olarak bulundu. PEG uygulanması için ana endikasyon olguların tamamında oral beslenememe ve gelişme geriliği idi. Yirmiyedi olgu nörolojik hastalık nedeni ile takip edilmekteydi. Bir olguda koroziv madde alımına bağlı özofagus darlığı mevcuttu. Olguların ağırlık ortalaması 10 persentilin altında idi. İşlem öncesi çekilen baryumlu pasaj grafisinde 4’ünde GER saptandı. Bir olguda PEG takılması sırasında teknik sorun gelişmesi üzerine cerrahi yöntemle gastrostomi açıldı. Geç dönemde 1 olguda PEG’in ciltaltına kayması nedeni ile revizyon yapıldı. Üç olguda PEG’in tıkanması sonucu değiştirildi. Olguların hiçbirinde işlem sonrası GER gözlenmedi. Ortalama 6 aylık takip döneminde olguların kilo artışlarının olduğu gözlendi.

Sonuç: PEG özellikle nörolojik problemleri olan olgularda uzun süre enteral beslenme desteğinin sağlanabilmesinde basit, etkili ve güvenilir bir yöntemdir.
