Laparoscopic-assisted full thickness abdominal wall repair of Morgagni hernia in children

 

AIM: Morgagni hernia is a rare entity of all surgically treated diaphragmatic hernias in pediatric age group. They are mostly asymptomatic and discovered incidentally. Open surgical repair has been the treatment of choice in all cases once diagnosed. However, since the introduction of minimal access surgery, different laparoscopic techniques of Morgagni hernia repair have been reported in the literatures. Herein, we report  the safety and efficacy of laparoscopic-assisted full-thickness anterior abdominal wall repair of Morgagni hernia in children.

 

METHODS: Four children with Morgagni hernias underwent primary laparoscopic repair by placement of interrupted 0 nonabsorbable sutures through the full thickness of the anterior abdominal wall, incorporating the hernia sac, the posterior rim of the defect, and returning back out through the anterior abdominal wall with an EndoClose device (Covidien) and  the sutures tied in the subcutaneous tissue. Demographics, presentation, operative findings, hospital stay, complications and follow-up were recorded.

 

RESULT: Between March 2007 and January 2011, 4 children with Morgagni hernia were treated laparoscopically in our institution. Male-female ratio was 3:1. Mean age was 6.25 years. The average operative time was  55 minutes. Average time to full feed was 20 hours. Postoperative analgesia requirement was minimal. The average hospital stay was 24 hours. All operations were completed laparoscopically. None of the patients developed intraoperative or postoperative complications. The maximum follow-up was 32  months. All patients are in good health without recurrence or significant sac residual.

 

CONCLUSION: Laparoscopic-assisted full-thickness anterior abdominal wall repair of Morgagni hernia  is a safe and effective treatment modality. It is well tolerated and gives all the advantages of minimal access surgery, Leaving the plicated sac in place does not cause any longterm adverse effects.

Çocuklarda Morgagni Hernisinde Laparoskopik Tam Kat Karın Duvarı Tamiri

 

Amaç:  Cerrahi tedavi gerektiren diafragma hernilerinden Morgagni hernisi çocuk yaş grubunda nadir görülür.Çoğunlukla asemptomatiktir ve tesafüden saptanır. Tercih edilen tedavi yöntemi önceleri açık cerrahi onarım olmuştur.. Ancak, minimal invaziv cerrahinin ortaya çıkması ile, Morgagni hernisi onarımında farklı laparoskopi teknikleri literatürde bildirilmiştir. Burada Morgagni hernisinde laparoskopik tam kat karın duvarı tamirinin etkinlik ve güvenilirliği tanımlanmıştır.

Yöntem: Dört hastada Morgagni hernisi tamiri laparoskopik olarak karın ön duvarından tam kat geçirilen , herni kesesi ve defektin posterior kenarını içeren ve Endoclose (Covidien) yardımıyla karın ön duvarından dışarı alınarak cilt altında bağlanan , emilmeyen O numara suturlar ile yapıldı. Yaş, semptomlar, ameliyat bulguları, hastanede kalış süresi, komplikasyonlar ve takip bulguları değerlendirildi.

Bulgular: Mart 2007- Şubat 2011 arasında 4 olguya Morgagni hernisi nedeni ile laparoskopik tamir yapıldı. Erkek-kız oranı 3:1 idi. Yaş ortalaması 6.25 olarak bulundu. Ortalama ameliyat süresi 55 dakika idi. Beslenmeye başlama süresi ortalama 20 saat olarak bulundu. Postoperatif analjezik gereksinimi minimal idi. Ortalama hastanede kalış süresi 24 saatti. Tüm operasyonlar

laparoskopik olarak tamamlandı. Olguların hiçbirinde intraoperatif ve postoperatif komplikasyon gözlenmedi. Maksimum takip süresi 32 aydı. Tüm olgular nüks veya belirgin rezidüel kese kalmaksızın iyileşti.

Sonuç: Morgagni hernisinde laparoskopik tam kat karın duvarı tamiri güvenli ve etkili bir tedavi yöntemdir. Bu yöntem iyi tolere edilir ve minimal invaziv cerrahinin tüm avantajlarını sunmaktadır.Yerinde bırakılan plike edilmiş herni kesesi uzun takipte olumsuz etkilere neden olmamaktadır.
