FOREIGN BODIES ASPIRATION IN CHILDREN

Aim:The young children have a tendency to place objects in their mouths. Therefore foreign bodies aspiration (FBA) is common in young children. The aim of this studyis to evaluate the results and complications of patients who underwent rigid bronchoscopy in the diagnosis of FBA.

Patients and methods:462 children (269 male, 193 female) underwent rigid bronchoscopy for FBA between 1992-2011 in our department. Age and gender of the patients, medical history, symptoms, physical examination, radiological and bronchoscopic findings were evaluated retrospectively. FBs were extracted using a rigid bronchoscope and suitable coaxiel forceps.

Results: The mean age of the patients is 3.1 years (1 months – 16 years) (Table I). Patients had symptoms like cough, wheezing, choking attacks,  respiratory distress. The duration of symptoms from the time of aspiration until bronchoscopy ranged from 1 hour to 2 years. The history in 349 patients (89.2%), physical examination in 246 (62.9%),  radiological findings in 286 (73.1%) were consistent with FBA. 84.6% (n:391) of patients  were foreign bodies. FBs were removed of 214 right, 147 left, 6 both mainstem bronchus and 24 trachea. Despite a history of aspiration, bronchoscopy was negative in 53( % 11.4 ) of the cases.Most of FBs were non-radio opaque and sunflower seeds and hazelnuts topped the list (Table II). Patients were evaluated with chest X-ray postoperative 3 hours and the patients were discharged the same day.

Tracheostomy and thoracotomy were performed in 2 and 4 patients respectively. Cardio respiratory arrest observed in 8 patients (2%) and 3 patients (0.7%) was died.

Conclusion:Diagnosis of foreign body aspiration in children is sometimes difficult. Therefore, bronchoscopy should be performed in children with apparent suspicion of FBA for the differential diagnosis and treatment. Family education is the best preventive measure for decreasing the incidence of FBA.

ÇOCUKLARDA YABANCI CİSİM ASPİRASYONLARI

Amaç:Tüm nesneleri ağzına götürme eğiliminde olan küçük çocuklarda yabancı cisim aspirasyonu (YCA) sık görülmektedir. Bu çalışmada, kliniğimizde YCA ön tanısıyla rijit bronkoskopi yapılan hastaların sonuçları ve komplikasyonları irdelenmiştir.

Hastalar ve metot:Kliniğimizde 1992-2011  yılları arasında 462 çocuğa (269 erkek, 193 kız) YCA ön tanısıyla rijit bronkoskopi uygulandı. Hastalar yaş, cinsiyet, öykü, semptom, fizik muayene, radyolojik ve bronkoskopik bulgular açısından retrospektif olarak irdelendi. Yabancı cisimler (YC) rijit bronkoskop ve uygun forseps kullanılarak çıkarıldı.

Bulgular:Hastaların yaş ortalaması 3.1 yıl (1 ay – 16 yaş) idi (Tablo I). Hastalarda öksürük, hırıltı, boğulma atağı, solunum sıkıntısı gibi semptomlar mevcuttu. Aspirasyondan bronkoskopi yapılana kadar geçen semptom süresi 1 saat – 2 yıl arasında idi. YC saptanan hastaların %89,2’sinde (n:349) öykü, %62,9’unda (n:246) fizik muayene bulguları, %73,1’inde de (n:286) radyolojik bulgular YCA ile uyumlu idi. Hastaların % 84,6’sında (n:391) YC saptandı. YC lokalizasyonları sıklık sırasına göre sağ ana bronş (n:214), sol ana bronş (n:147), trakea (n:24) ve her iki bronş (n:6) idi. 53 hastada aspirasyon öyküsü olmasına rağmen bronkoskopi negatifti (%11.4).Aspire edilen yabancı cisimlerin çogu radyolusent olup, en çok aspire edilen maddeler ay çekirdeği  ve fındık idi (Tablo II). Hastalar postoperatif  3. Saat AC grafisi ile değerlendirildi ve hastalar aynı gün taburcu edildi.

2 hastada trakeostomi ve 4 hastada torakotomi uygulandı. 8 hastada (%2) kardiyopulmoner arrest gelişti ve toplam 3 hasta (%0,7) kaybedildi.

Sonuç:Çocuklarda YCA’nun tanısı bazen zordur. Bu nedenle YCA şüphesi belirgin olan çocuklarda ayırıcı tanı ve tedavi için bronkoskopi  yapılmalıdır. Aile eğitimi YCA sıklığının azaltılmasında en etkili koruyucu önlemdir.
