NOTICE TRACHEAL ATREZIA THAT CAN NOT BE TRACHEAL INTUBATION IN NEWBORNS

Introduction and aim: Tracheal atresia is a rare congenital anomaly. Clinically, the finding in the newborn, respiratory distress and unable to cry that the biggest problem in these patients can not be intubated. Presented a patient that born in our hospital with the diagnosis of tracheal atresia after the tracheostomy and esophagoscopy.

 

Case Report:  A male infant weighing 2200 gr that  prenatally not follow was delivered after 40 weeks of gestation by vaginal delivery at our hospital. The baby was interned at neonatal intensive care unit due to respiratory distress and cyanosis . After the failure of the many attempts to tracheal intubation, consultation with pediatric surgery; at the level of cricoid cartilage of the larynx was obstructed full are seen by laryngoscope  and an emergency tracheostomy was performed. There was no tracheal cartilage in the neck dissection,  during bag-mask ventilation expanding esophagus was intubated by esophagostomy. Blood saturation of the patient improved and inspiration of the the lungs were better. A chest x-ray  both of lungs are expanded. There was no tracheal opening at the emergency esophagoscopy. Only 2.7 mm 0 degrees telescope were advanced to fistula is a tiny mouth on the front at the level of middle esophagus, after the trachea carina begins about 1.5 cm. Tracheal atresia is thought to be Type II Floyd (or E-type tracheal atresia of Faro) intubated patient with also nasogastric decompression  taken into intensive care. After the progression of respiratory distress, he died at postnatal 12thhour. Autopsy did not accepted by the family.

 

Conclusion: When the rise in saturation  by bag-mask ventilation cases, tracheal atresia should be considered that postpartum sudden onset of respiratory distress, cyanosis, and  no audible cry. Despite esophageal intubation and gastric decompression remains a deadly disease. Although survival can not be reported in the literature by surgical operations, the development of tissue engineering in reconstructive operations after the production of cartilage tissue suggest that long-term survival could be achieved. 

ENTÜBASYON YAPILAMAYAN YENİDOĞANLARDA TRAKEAL ATREZİYE DİKKAT

Giriş ve Amaç: Trakeal atrezi oldukça nadir görülen konjenital bir anomalidir. Klinik olarak yenidoğanda solunum sıkıntısı ve afoni olarak bulgu veren bu hastalarda en büyük problem entübe edilememeleridir. Trakeostomi ve özofagoskopi sonrası trakeal atrezi tanısı alan bir olgu sunulmuştur.

 

Olgu Sunumu: Prenatal takipsiz olup 40.hafta, normal vajinal yolla hastanemizde doğan, 2200 gr ağırlığında erkek bebek solunum sıkıntısı ve siyanoz gözlenmesi üzerine yenidoğan yoğun bakım ünitesinde interne edildi. Birçok trakeal entübasyon girişimlerinin başarısızlıkla sonuçlanması üzerine yapılan çocuk cerrahisi konsültasyonunda; laringoskop ile larinksin krikoid kıkırdak seviyesinde tam obstrükte  olduğu görüldü ve acil trakeostomi yapıldı. Boyun disseksiyonunda trakeal kıkırdak dokusu görülmedi, ambulama esnasında genişleyen özofagustan özofagostomi yapılarak entübe edildi. Hastanın saturasyonlarının düzeldiği ve akciğerlerin daha iyi havalandığı görüldü. Akciğer grafisinde her iki akciğerin havalandığı görüldü. Acil özofagoskopide trakeal açıklık görülmedi. Orta özofagus seviyesinde ön yüzdeki minik fistül ağzından sadece 0 derece 2.7 mm’lik teleskop ilerletilebildi, yaklaşık 1.5 cm’lik tarkedan sonra carinanın başladığı ve her iki bronşun başladığı görüldü. Floyd Tip II Trakeal atrezi (Faro E tipi Trakeal atrezi) düşünülen hasta ösefagustan entübe halde ve ayrıca nazogastrik dekomprasyon ile yoğun bakıma alındı. Postnatal 12. saatte solunum sıkıntısının ilerlemesi sonrası exitus oldu. Aile otopsiyi kabul etmedi.

 

Sonuç: Doğum sonrası ani başlayan solunum sıkıntısı, siyanoz ve afonili olgularda ambulama ile saturasyonlarda yükselme olduğunda trakeal atrezi düşünülmelidir. Esofageal entübasyon ve gastrik dekompresyona rağmen hastalık ölümcül seyretmektedir. Literatürde yapılan cerrahi operasyonlarda sağkalım sağlanamamış olmasına rağmen, doku mühendisliğinin gelişmesi sonrası kıkırdak doku üretimi ile yapılacak rekonstruktif operasyonlarla uzun süreli sağkalım sağlanabileceğini düşünmekteyiz.
