Isolated Paracardial Cyst: Prenatal Diagnosis and Intervention with Postnatal Early Exploration

Introduction: Isolated  paracardial  cysts,  defined  as  cystic structures  adjacent  to  or  originating  from  the  heart, are   rare   and   etiologically   heterogeneous   congenital abnormalities. These lesions have a wide spectrum of anomalies extending from neuroenteric cysts to thmic cysts. Paracardial cysts are uncommon prenatal findings and only  a  handful  of  similar  cases  have  been  reported in  the  medical  literature with prenatal interventions and postnatal early explorations.

Patient: The 26 weeks old fetus of a 31 year-old healthy mother was diagnosed to have a 3x2 cm paracardial cyst in the left anterior mediastinum and percutaneous cyst aspiration was performed. As the cyst was found to be enlarging and exerting pressure on the fetal lung on the later weeks of the pregnancy by ultrasound examination, fetal magnetic resonance imaging was performed. The presumptive radiological diagnosis was  a thmic cyst. Percutaneous aspiration of the cyst was performed once more on the 31st gestational week and the patient was delivered with a planned sectio on the 35th weeks of the gestation. As the patient had severe dyspnea during his follow–up in the neonatal intensive care unit, he was taken to operation. A left sided thoracotomy revelaed a 3 cm diameter cystic structure adcacent to the pericardium and originating from the timus. Thus, partial excision of the csyt with partial excision of the left lobe of the timus was performed. Histopathological examination of the cyst was consistent with a thymic cyst. The patient developed left diaphragmatic paralysis and he underwent a diaphragmatic plication.

Result:The patient is stil followed in the neonatal intensive care unit 70 days after the first and 20 days after the second operation due to pneumonia.

Discussion: Congenital paracardial cysts have a wide spectrum of anomalies. Although, these lesions are known to regress during the last weeks of the gestation the cyst in the present patient did not. Prenatal diagnosis of a paracardial cyst gives the surgeon opportnity of early planning of an operation in a dyspneic newborn in the early postnatal period.

İzole Parakardial Kist: Prenatal Tanı-Girişim ve Postnatal Erken Eksplorasyon

Giriş: Parakardial izole kistler, kalbe komşu veya kalp dokusundan kaynaklanan doğumsal lezyonlardır. Bu lezyonlar, nöroenterik kistlerden timik kiste kadar uzanan geniş bir grubu kapsarlar. Bahsedilen lezyonlarda prenatal tanı ve girişim ile doğum sonrası erken dönemde operasyon yapılması literatürde son derece nadirdir.

Hasta: Otuz-bir yaşında sağlıklı annenin ilk gebeliğinden olan erkek fetusta 26. gestayon haftasında yapılan fetal ultrasonografide sol anterior mediastende 3x2 cm boyutunda parakardial bir kist saptanarak perkütan kist aspirasyonu yapılmış idi. Gebeliğin ilerleyen haftalarında kistin tekrar büyüdüğü ve komşu akciğer dokusuna baskı yaptığı fetal ultrasonografi ile görülerek hastaya fetal manyetik rezonans görüntüleme yapıldı. Ön tanı olarak lezyonun timik kist olabileceği düşünülerek 31. gestasyon haftasında kist bir kez daha aspire edildi. Otuz-beşinci gestasyon haftasında planlı sezaryen seksiyo ile doğurtulan hastanın yenidoğan yoğun bakım ünitesindeki takibinde solunum sıkıntısı gelişmesi üzerine hasta operasyona alındı. Sol torakotomi ile anterior mediastende perikard komşuluğunda ve timus kaynaklı olarak görülen kist timusun sol lobunun bir kısmı ile beraber parsiyel olarak çıkartıldı. Histopatolojik inceleme sonucunda rezeke edilen dokunun timik kist olduğu doğrulandı. Takiplerinde sol diafragma paralizisi gelişen hastanın diafragma pilikasyonu yapıldı.

Sonuç: İlk operasyondan 70, ikinci operasyondan 20 gün sonra hasta yenidoğan yoğun bakım ünitesinde pnömoni nedeniyle ekstübe olarak izlenmektedir.

Tartışma: Doğumsal parakardial kistler geniş bir yelpazeye sahip kistik oluşumlara verilen bir isimdir. Bu lezyonların birçoğu kendiliğinden gerilemesine karşın bahsedilen olguda kistin boyutunda küçülme olmamıştır. Doğum öncesinde parakardial kist saptanmış ve post-natal erken dönemde solunum sıkıntısı gelişen hastalarda prenatal tanı cerrahinin erken planlanmasına olanak vermektedir.
