Introduction: Pierre-Rubin Syndrome (PRS) is characterized with the triad of  micrognathia, glossoptosis and cleft palate . In this report, a case with oropharyngeal web, hypoplasia of the mandibula and the tongue is presented.

Case: The female baby who was given term birth with a birth weight of 3.200 gr as the first offspring of the seventeen years old mother from her first pregnancy was referred to our clinic because of the the observation of lingual hypoplasia and micrognathia at her physical examination. A tracheostomy was performed  because of the impossibility of intubation of the patient who was begun to be followed with the diagnosis of PRS, who had an oropharyngeal web and who had severe retractions and respiratory distress. The oropharyngeal web was excised and gastrostomy  was opened. Mandibular distractors were placed by the mandibular surgery staff and the gap between their arms  were widened  everyday after the fifth postoperative day. At her follow up, maxillofacial computerized tomography (CT) showed that mandibular development was normal. She had an adequate opening and her tracheostomy was closed nine months after it had been established.

Result: PRS is an autosomal recessive disorder which is characterised by micrognathia, glossoptosis and cleft or high arched palate. The chief problem that increases the severity of the clinical presentation is respiratory distress. Plenty of therapeutic maneuvers were performed to stabilize upper airways in these patients; the use of mandibular osteogenesis distractors to elongate the mandibular rami is the newest and mostly used of them. Our patient had an oropharyngeal web in addition to the routine clinical features of PRS. Our patient did not have cleft palate; but had micrognathia, lingual hypoplasia and oropharyngeal web. Mandibular retractors were used bilaterally in our patient and were followed up with maxillofacial CT. The use of mandibular osteogenesis distractors, which is claimed to be effective at the literature decrease the need for tracheostomy or even discard it totally in some patients

Giriş: Pierre Robin Sendromu; mikrognati, yarık damak ve glossoptozis triadı ile karakterizedir. Burada mandibula hipoplazisi, orofarengeal web ve dil hipoplazisi olan bir olgu sunulmuştur.

Olgu:17 yaşındaki annenin 1. gebeliğinden 1.yaşayan olarak miadında 3200 gr olarak doğmuş kız mikrognati ve dil hipoplazisi saptanması sonrası tarafımıza yönlendirildi. Pierre Robin sendromu ön tanısıyla izleme alınan hastanın ciddi retraksiyonları ve solunum sıkıntısı olması nedeniyle girişilen entübasyon çabasının başarısız olması üzerine trakeostomi yapıldı ve orofarinksi tamamen kapatan web açıldı. Gastrostomi yapıldı. Çene cerrahları tarafından bilateral mandibula distraktörleri yerleştirilen hastanın işlemden 5 gün sonra distraktörler hergün açıldı. Daha sonra maksillofasial tomografi ile kontrol edilen hastanın çene gelişiminin iyi olduğu görüldü. Orofaringeal açıklığı iyi olan hastanın 9. ayda trakeostomisi kapatıldı.

Tartışma: Pierre Robin Sendromu; mikrognati, glossoptozis ve yarık damak veya yüksek damakla karakterize otozomal resesif bir hastalıktır. Bu hastalarda kliniği ağırlaştıran esas problem solunum sıkıntısının olmasıdır. Olgumuzda buna ek olarak bulunan orofaringeal web olması literatürde bulunmamaktadır. Tanımlanan olguda yarık damak olmamasına karşın mikrognati, hipoplastik dil ve beraberinde orofaringeal web eşlik etmekte idi. Bu hastalar için üst havayolunu stabilize etmek için çeşitli terapatik manevralar denenmiş. İçlerinde en yeni ve sık uygulanan mandibular osteogenezis distraktörleri olmakla beraber olgumuza da bilateral mandibular distraktörler yerleştirildi. Distraktörler maksillofasial tomografiler ile kontrol edildi. Litaretürde de efektif olduğu gösterilmiş olan bu distraktörler hastaların trakeostomi ihtiyacını azaltıp bazılarında ortadan kaldırmaktadır
