The Presentation Of A Rare Ectopic Testis: Penopubic Testis

Background/Aim: The incidence of cryptorchidism is 1-4.6% of all full-term male infants. About 1.9% of these are ectopic testes. The most common position is in the superficial inguinal pouch, sometimes an ectopic testis can be identified in a femoral,perineal,pubic,penile or contralateral position. Cases of ectopic testis located in the dorsal part of the penis,in the mons pubis,are rare.Here,it is aimed to present a case of ectopic testis located in the penopubic area.
Case: A three-year-old boy presented with the complaint of left testis not being in scrotum. The right testis was located in the scrotum, the left testis was ectopically located on the dorsal penis, and distal hypospadias was present. Orchiopexy was planned.Laparoscopy was initiated to evaluate the anatomy of the cord and vessels. In laparoscopy,the right internal ring was closed,vas deferens and vessels were entering through the internal ring. The vas deferens and vessels of the left testis were also entering from the left internal ring. Laparoscopy was ended and left inguinal exploration was started. The funiculus was suspended and the ectopically located testis was reached. The testis was freed from the surrounding tissues and gubernaculum. Gubernaculum was observed to progress towards the penopubic area. The testis was placed into the scrotum.
Discussion: Penile or penopubic ectopic testis is a rare pathology. In the literature,presentations with absence of testis,pain,penile mass in the scrotum have been reported.It is seen in the publications that the vas and vessels of the testis are of sufficient length.
Conclusion:Ectopic testis located in the penopubic area is a rare pathology. Laparoscopy may be a good start to see the patway of the vas deferens and vessels. These testes can be easily moved into the scrotum by performing classical orchiopexy steps with inguinal exploration.

Ön bilgi/Amaç: Kriptorşidizm insidansı genel popülasyonda %1-4.6 kadardır. Bunların %1.9 kadarı ektopik yerleşimli testislerdir. En sıklıkla superfisyal inguinal poşta olmak üzere, femoralde, perinede, pubik, kontralateral tarafta yerleştiği görülür. Penisin dorsalinde, mons pubiste yerleşmiş ektopik testis olguları ise çok çok nadir görülür. Burada penopubik alanda yerleşmiş bir ektopik testis olgusunun sunulması amaçlanmıştır.
Olgu: Üç yaş erkek hasta sol testisin yerinde olmaması şikayeti ile polikliniğe başvurdu. Yapılan fizik muayenede sağ testisin skrotumda olduğu, sol testisin penis dorsalinde ektopik yerleşimli olduğu, distal hipospadiası olduğu görüldü. Orşiopeksi yapılması planlandı. Öncesinde kord ve damarların anatomisini değerlendirmek için laparoskopi ile başlandı. Laparoskopide sağ internal ring kapalıydı, vas deferens ve damarlar internal ringden içeri giriyordu. Sol testisin vas deferens ve damarları da sol internal ringden içeri giriyordu. Laparoskopi sonlandırılarak sol inguinal eksplorasyona geçildi. Funikulus askıya alındı ve ektopik yerleşimli testise ulaşıldı. Testis çevre dokulardan ve gubernakulumdan serbestleştirildi. Gubernakulumun penopubik alana doğru ilerlediği görüldü. Yüksek ligasyon ve herniyotomi yapıldı. Testis skrotuma indirildi.
Tartışma: Penil veya penopubik ektopik testis oldukça nadir bir patolojidir. Literatürde skrotumda testis yokluğu, ağrı ve penil kitle ile başvurular bildirilmiştir. Bizim vakamızda olduğu gibi, testisin vas ve damarlarının yeterli uzunlukta olduğu, yapılan yayınlarda görülmektedir.
Sonuç: Penopubik alanda yerleşmiş ektopik testis oldukça nadir görülen bir anomalidir. Vas deferens ve damarların ilerleyişini görmek amacıyla laparoskopi yapılması iyi bir başlangıç olabilir. Bu vakalar inguinal eksplorasyonla klasik orşiopeksi basamakları uygulanarak kolaylıkla indirilebilir.
