The aim of the present study is to investigate the characteristics of our cases which ureteroneocystostomy operation performed by Gleen- Anderson technique and to specify the characteristics of these cases and the efficiency of this technique.

10 cases which are operated by Glenn-Anderson technique between 2009-2013 years were taken into this study. Of our cases 2 were girls and 8 were boys. The youngest patient was 20 moths-old and the oldest patient was 16 years old. The mean of our patients was 5.2 years.  Glenn-Anderson technique was performed to 8 cases ureterovesical obstruction  (UVO)and 2 cases vesicoureteral reflux with associated  paraureteral diverticulosis.  4 obstruction cases had left UVO, 1 cases had right UVO, 3 cases had bilateral UVO. Of the patients who are operated with the diagnosis of UVO , 4 cases had tapering ureteroplasty, 1 cases star plication and 3 cases had no  ureteral tapering operation. The mean ureter diameters measured was 22mm (15-40mm). In follow-up no recurrent reflux or obstruction was seen in any of the cases. In retrospective evaluation of our patients we observed that , Glenn- Anderson technique can be performed in over dilated ureters due to UVO and VUR cases,  In this technique blind extravesical dissection is not necessary.

In conclusion,  Glenn-Anderson method should be the preferred method that can be performed efficiently, secure and successfully in  cases especially the ones that have dilated ureters the need bladder reimplantation thus necessitate tapering.

Çalışmamızın amacı anabilim dalımızda Glenn-Anderson tekniği ile üreteroneosistostomi operasyonu uygulanan olguların özelliklerinin araştırılması ve bu  yöntemin tercih edildiği olguların özelliklerinin belirlenmesi ve yöntemin etkinliğinin değerlendirilmesidir.

2009-2013 yılları arasında Glenn-Anderson tekniği ile  opere edilen 10 olgu çalışmaya dahil edilmiştir. Olgularımızdan  2si kız, 8 olgu erkektir. En küçük olgu 20 aylık, en büyük olgu 16 yaşındadır. Yaş ortalaması 5.2 yıldır. Bu yöntem 8 olguda, üretero vezikal darlık(UVD), 2 olguda  ise veziko üreteral reflü(VUR) ve eşlik eden  paraüreteral divertikül nedeniyle uygulanmıştır.  Darlık saptanan olgularımızdan 4 olguda sol UVD, 1olguda  sağ UVD, 3 olgu bilateral UVD mevcuttur. Ureterovezikal darlık nedeniyle opere edilen olgulardan 4 olguya tapering üreteroplasti, 1 olguya star plikasyon yapılırken, 3 olguda üreteral daraltma işlemi uygulanmamıştır. VUR  ve paraüreteral divertikül nedeniyle opere edilen olgulardan birinde tapering ureteroplasti yapılırken diğerine gerekmemiştir. Olguların operasyon sırasında ölçülen üreter çapları  40mm -15 mm (ortalama 22mm)  saptanmıştır. Olguların hiçbirinde takip sırasında  tekrarlayan reflü veya darlık görülmemiştir. Olgularımızın retrospektif değerlendirilmesi sonucunda Glenn-Anderson yönteminin  UVD veya VUR nedeniyle aşırı dilate olmuş üreterlerin  intravezikal  diseksiyonu sırasında orijinal hiatus genişlemesi nedeniyle tercih edildiği saptanmıştır. Bu teknik seçildiğinde, kör bir ekstravezikal girişime gerek kalmamaktadır.  2 olgumuzda  orijinal hiatus mesane duvarı mediolaterale doğru direkt görüş altında  tam kat kesilerek  tunelin uzatılması gerekmiştir.

Sonuç olarak, bu yöntem ile kör ekstravezikal girişime gerek olmaksızın yeterli uzunlukta bir submukozal tunel oluşturulabilmiştir. Glenn-Anderson yöntemi UVD ve VUR nedeniyle özellikle  tapering gerektirecek kadar dilate ureterlerin mesaneye reimplantansyonunda  tercih edilebilecek etkin ve güvenli ve başarılı bir yöntem olarak değerlendirilmiştir.
