TESTICULAR PROSTHESIS IN CHILDREN

Aim: Testicular prostheses are inserted for cosmetic and psychologic reasons for absent testes or after an  orchidectomy in children. Aim of this study is to  review the insertion of paediatric testicular prosthesis at our institution over last 5 years and to discuss a safe protocol for insertion.

Patients and Method: From 2008 to 2012, 15 patients aged between 2 and 14 years with a mean age of 11 years had testicular prostheses implanted. Nine patients had right, and six patients had left testicular prosthesis. The most common indication for surgery was  vanishing testes. Eight patients presented with vanishing testis, and six patients presented with atrophy after orchidopexy or inguinal hernia repair. Small or medium size silicone gel prostheses were used. Each prosthesis was implanted through an inguinal skin incision, inserted digitally, and fixed with purse-string sutures.

Results: Two patients had extrusion after placement and one patient had wound infection necessisating prosthesis removal.  All patients were happy with the implantation of prosthesis at the follow-up.

Conclusion: The use of testicular prosthesis in children is increasing. Small or medium sized silicone gel prostheses can be placed safely in children.  They may be replaced with a larger size if needed. Groin route should be preferred as it is associated with fewer complications.

 

ÇOCUKLARDA TESTİS PROTEZİ

Amaç: Çocuklarda testis protezi testis yokluğunda ya da inguinal cerrahi sonrası atrofi gelişmesi durumlarında kozmetik ya da psikolojik nedenlerle uygulanan bir işlemdir. Bu çalışmanın amacı 5 yıllık sürede testis protezi yerleştirilen hastaları takdim etmek ve güvenli uygulama ile ilgili hususları tartışmaktır.  

Hastalar ve Metod: 2008-2012 yılları arasındaki beş yıllık dönemde yaşları 2 - 14  yaş arasında değişen 15 çocuğa testis protezi yerleştirildiği tesbit edildi. Yaş ortalaması 11 yıl olan çocuklarda 9 hastada sağ, 6 hastada sol testis protezi yerleştirilmişti. Hastaların sekizi vanishing testis ile başvurmuştu. Dört hasta ise daha önce inmemiş testis nedeni ile orşidopeksi yapılan fakat atrofi gelişen hastalardı. Bir hasta inguinal herni, bir hasta ise hidrosel nedeni ile ameliyat olup daha sonra testis atrofisi gelişen hastalardı. Hastalara küçük ya da orta boy silikon jel testis protezi yerleştirilmişti. Cerrahi olarak inguinal kesi ile parmak yardımı ile oluşturulan tünelden ve skrotumdaki boşluğa protezin yerleştirilmesi ve proksimalin de çevre sütürü ile kapatılması yöntemi uygulanmıştı.

Sonuçlar: İki  hastada skrotum cildinin erozyonu ve sonrasında  da cilt açılması nedeni ile protezin  çıkarıldığı, bir hastada ise yine enfeksiyon sonrası cilt açılması nedeni ile protezin çıkarıldığı tesbit edildi. Diğer hastalarda protezle ilgili bir sorunun yaşanmadığı ve psikososyal olarak mutlu oldukları tesbit edildi.

Sonuç: Çocuklarda testis protezi son yıllarda daha sık uygulanan bir işlem olmaktadır. Çocuklarda küçük ya da orta boy silikon jel protezler güvenle yerleştirilmektedir. Gerektiğinde ileri yaşlarda daha büyükleri ile değiştirilebilirler. Daha az komplikasyon görülmesi nedeni ile inguinal yol tercih edilmelidir.
