Aim:

The evaluation of testesteron oilment therapy for penil enlargement results before hypospadias surgery in our clinics.

Material Method:

We evaluated the reports of 70 children who had distal hypospadias surgery in our clinics between Jan 2008 and Jan 2013. We prescribed local testesteron (dihydrotestesteron gel) once daily for one month to 18 children who had insufficient urethral plate before the surgery. All surgeries performed as Snodgrass procedure over silicon catheter which is kept post operatively. The follow up were 3 days, 1 month and 1 year post operative.   

Results:

The preoperative and postoperative results of 18 children used testesteron gel and 52 children who had not used it were compared. After one month of testesteron gel treatment pubic hair was seen in 3 children. The operations of children used testesteron gel were easier as creating neourethra and supportive tissue. The operation time for gel treatment group was 82 minutes (52-126) while it was 106 minutes (82-154) in non treatment group. During the operation bleeding was more problem in children who used testesteron gel and needed dressing for wound every day. The results of long term follow up are 4 (%22) fistula formation in testesteron gel group and 4 (%7,6) fistula formation in non testesteron treatment group. Meatal stenosis was seen in 1 child (%5,5) in the testesteron treatment group and in 3 (%5,2) child who had not used testesteron gel.

Conclusion:

The preoperative testesteron oilment therapy is prefered for making the surgery more easier and shorter, but it increases the risk of post operative bleeding and fistula formation.  

Amaç:

Kliniğimizde hipospadias tamirlerinde, ameliyat öncesi penis büyümesinin sağlanması amacı ile lokal testesteron krem tedavisi uygulanan olguların sonuçlarının değerlendirilmesidir.

Gereç ve Yöntemler:

Ocak 2008 ile Ocak 2013 yılları arasında kliniğimizde distal hipospadias tanısı ile ameliyat edilen 70 olgunun dosyaları geriye dönük olarak incelendi. Ameliyat öncesi üretral plağın yeterli genişlikte olmadığı düşünülen 18 olguya, bir ay süresince günde bir kez lokal testesteron  (dihidrotestesteron jel) uygulanmıştır. Hastaların tümüne silikon kateter üzerinden Snodgrass tamiri yapılmış ve ameliyat sonrası kateterli takip edilmişlerdir. 3 gün, 1 ay ve 1 yıl sonra rutin kontrolleri yapılmıştır.

Bulgular:

Testesteron uygulanan 18 olgu ve uygulanmayan 52 olgu operasyon öncesi ve sonrası bulguları yönünden karşılaştırılmıştır. Bir aylık lokal testesteron uygulaması sonrası 3 çocukta pubik kıllanma görülmüştür. Operasyon süresince krem kullanan olgularda neoüretra ve üzerine getirilecek çevre dokular daha kolay tamir edildi. Krem kullananlarda operasyon süresi ortalama 82 dakika (52 – 126) iken, kullanmayanlarda 106 dakika (82–154) idi. Operasyon sonrasında krem kullanan olgularda kanama daha fazla gözlendi ve her gün pansuman tekrarı gerekti. Uzun dönem takiplerinde krem kullanan 4 olguda (%22), kullanmayan 4 olguda (%7,6) fistül geliştiği görülmüştür. Meatal darlık krem kullanan 1 olguda (%5,5), kullanmayan 3 olguda (%5,2) görülmüştür.

Sonuç:

Hipospadias cerrahisi öncesi testesteron krem kullanımı cerrahi işlemin kolaylığı ve süreyi kısaltması nedeni ile tercih edilmekle birlikte, ameliyat sonrası kanamada artış ve uzun dönemde fistül gelişme oranını yükselttiği düşünülmektedir.
